Does Your Parent or Family Member Have Two Or More Chronic Conditions?

Has their physician enrolled him/her in Medicare’s groundbreaking program developed to reduce patient hospitalizations?

 If you are not a medical professional, it is worrisome trying to monitor the health of your parent. Sometimes, parents are confused and ashamed to ask questions of their doctors and nurse practitioners. They are afraid of appearing frail and incompetent. Maybe, they simply misunderstood their doctor’s instructions. 

 What if your parent felt comfortable sharing their concerns, tribulations and questions with a certified health care professional every month? What if that person was the liaison between the patient and the doctor? What if that monthly phone call prevented one emergency room or hospital visit?

In 2015 Medicare introduced their Chronic Care Management program for anyone with two or more significant chronic conditions that are expected to last 12 months or more.* Here is just a short list of examples:

Asthma

Diabetes

Cancer

Chronic Obstructive Respiratory Disease

Atrial Fibrillation

Hypertension

Cardiovascular Disease

Arthritis

Osteoporosis

Dementia

Alzheimer’s

Autism Spectrum Disorders

Depression

Chronic Kidney Disease

Infectious Diseases such as AIDS

(Note: Patients under nursing care are not eligible for this program.)

Do your parents visit multiple physicians?

Are they all prescribing medications? Do you know whether your parents are taking their medications because 20 – 30% of Medicare patients never take prescribed medication? Do you know if there is a conflict between any of the medications? What if you knew that a dedicated registered pharmacist was reviewing all of the medications that your parent was taking or prescribed to take?

Would a program give you some peace at night that in-between regular office visits, a certified medical professional was speaking with your parent, collecting medical information from all health care providers and recording all of this in a centralized database accessible to any health care provider that your parent and the physicians want to have access?

Would you rest easier or concentrate better on all of your other equally important responsibilities knowing that someone else is checking that your parent is following the physician’s orders?

Would you breathe just a little easier knowing that if a change in their medical condition is identified, the physician would be alerted to try and prevent a hospitalization?
