Retrograde ejaculation occurs when the seminal fluid is ejaculated from the posterior urethra into the bladder. This is caused by abnormal function of the internal sphincter of the urethra and can stem from anatomic disruption, lymph node invasion, or DM. The condition can also result from interference with the sphincter function from drugs such as alcohol, antipsychotics, antidepressants, and antihypertensive agents. Retrograde ejaculation can be treated with dextroamphetamine, ephedrine, phenylpropanolamine, and pseudoephedrine. The evaluation should include a detailed history of the patient’s sexual problems, medications, and relationship quality.
