Digitalization in Healthcare: A Firsthand Account

???? ???? As a practicing neurologist and pioneer of digitalization, I have over 20 years of experience in this field. Alongside a team, approximately 25 years ago, I initiated the first database for dementia patients. For the past 15 years, I've been the CEO of #NeuroTransData and #NeuroTransConcept, while also working as a neurologist with 4 other colleagues in a practice - so, I'm familiar with both sides!

?? Our work at NeuroTransdata (NTD) and NeuroTransConcept (NTC) focuses on capturing Real World Evidence (RWE) data, from which we've developed tools for doctors, medical assistants, and patients. Our platform #DESTINY offers interactive features for doctors and patients - a dream of the digitalization scene that we're already living! ?? We have a high approval rate (> 90%) for handling patient data.


So, why is digitalization still a challenge in the medical profession?


?? Unclear benefits: The benefits of transitioning are often not clear to practices. lack of information about the possibilities of AI tools.

?? AI anxiety: The fear of being replaced by Artificial Intelligence.

?? Resistance to change: People, including doctors, are reluctant to leave their comfort zones, especially without pressure or necessity.

?? Financial dependencies: Doctors receive their payment from KV (Association of Statutory Health Insurance Physicians) and partly from private insurances, which provides little incentive for change unless these institutions demand it or compensate adequately.

?? Prepayment: Few doctors are willing to prepay. Why should they? It means additional effort and costs.

?? Budget constraints: Unlike hospitals, practices do not receive governmental compensation for inflation or deficits. Only what is earned can be invested.

??? Guiding institutions: KVs and professional associations, elected by doctors, often delay innovations to avoid "hurting" anyone - the representatives are elected by all doctors

Revolutions ???, Reforms ???, and Evolutions

?? Revolutions, by definition, always come from below when large portions of the population - in our example, the patients - are dissatisfied with a system. The gradual system change from within takes the longest (Evolution), with reforms being an intermediate stage but can be indefinitely delayed through clever lobbying. Let's see how it goes. In any case, all players MUST come together and avoid mutual "bashing".... that's just how it is in a democracy.

The goal? Better, patient-centered care through meaningful digitalization. ??????????

Arnfin Bergmann MD very often practical limitations are due to double or triple documentation including the local database at practice (Praxisverwaltungssystem - PVS) and the real-world database itself. If hospital data or smartphone-data are available addititional data capturing is necessary - usually done manually. In our #neurotransdata network most of this work is done by our specialized study nurses and study coordinators, human ressources of limited availability. I believe that in future we have to automate these time consuming processes by #ai. In our #neurotransresearch-App patient reported outcomes (#PROM) and passive digital biomarkers (#pdb) can only be the first step... Kick-off very soon ??
