A very important lesson that we should all note about the CEO of Oceangate, Richard Rush, who was himself on the submarine and paid with his life for the mistakes of his employees in the company.

Advice to every CEO or president of a company that is aware of ignoring or neglecting your team in place.
Create an environment of physical safety that allows for the free exchange of ideas
Don't help the team that just agrees with your ideas and doesn't do it challenge and is afraid to speak and object.
Leadership mistakes are permissible in smaller companies, and they will not appear in this way, but the result is the same: choosing your team can cost you a lot.
