Aliansa DC-3T Accident at San Felipe
An Aliansa Douglas DC-3T, registration HK-5016, veered off the dirt runway during landing at San Felipe, Colombia, on April 8, 2022. The aircraft, arriving from Inirida with six people on board, experienced a right main gear strut collapse but reported no injuries. Preliminary information suggests a tyre/wheel failure caused the aircraft to veer left off the runway. Colombia's Aerocivil confirmed there were no injuries and that an investigation by the GRIAA is underway to determine the exact cause of the incident. Click image above for reading the full article!
Maintenance Failures Found in Gazpromavia SU95 Crash Investigation
On July 29, 2024, it was revealed that maintenance failures contributed to the crash of Gazpromavia Sukhoi Superjet 100-95, registration RA-89049, near Moscow on July 12, 2024. Inspections of the Lukhovitsy maintenance facility found incomplete checks on rudders and elevators, unqualified supervision, and other violations in the C-Check process for the crashed aircraft and another SU95. As a result, Rosaviatsia has ordered Gazpromavia to stop using the second SU95, tail number RA-89029, highlighting serious concerns about maintenance practices that led to the fatal crash of three crew members. Helicopter Crash Investigation in Fukuoka, Japan
A Robinson R44 Raven II helicopter operated by SGC Saga Aviation crashed in Shonan-machi, Yanagawa, Fukuoka, Japan, killing both the pilot and engineer on board. The helicopter, which had been conducting sightseeing flights, crashed on approach to Saga Airport after departing from Hita city, Oita prefecture. The Japan Transport Safety Board has initiated an investigation to determine the cause of the crash, which resulted in the aircraft being completely destroyed and the tail section being found hundreds of meters from the main wreckage.
