Create 'DaD not MoM'

What do we wait for when a meeting end ? what do we create with great precision, sometimes let the moment pass by not living in reality, but planning for future - a Minute of Meeting (MoM) document.
This single piece of document is thought to the anchor of all future action and hold all accountabilities listed with great detail, encapsulating a great plan for the future.
Reality takes to shape a few minutes later after we receive the 'MoM', it gets read and then tossed into a folder, to be taken out for the next meeting and the realization hit that no action has been taken or the whole understanding of the concept was wrong or info capturing, analysis and processing done by someone who just wrote it and never realized what it was supposed to be meant for. Ask yourself who wrote the MoM, mostly it is done by a junior staff who is always thought an unnecessary participant in a meeting but always present to take note, how that person is then accountable to capture the crux of the matter that becomes the root documentation of all future action.
Let's introduce 'DaD' -Detailed Accountability Dossier. This document does not get created after the meeting end in seclusion but during the meeting itself, after each topic is closed, Accountable person for the task, writes down what needs to be done, how it will be done and what are possible outcomes with all stated risk involved.
This single act brings clarity and accountability of highest order with a clearly defined set goal to how to achieve the target. It also bring the timelines in perspective, I personally have seen 10x improvement in task execution once DaD methodology is implemented.
So key difference between 'MoM' and 'DaD' is to let Task holder do all the documentation and understand the risk, deadlines, and deliverable in the meeting itself and not let a third party do it for you.
