Correlation between physiology and anatomy

The correlation between invasive functional testing (hyperemic/FFR and non-hyperemic/iFR/RFR/DFR, etc) and intravascular imaging findings ([IVI] intravascular ultrasound or optical coherence tomography) remains quite controversial. The minimal luminal area (MLA) measured by IVI may be the most reliable indicator of myocardial ischemia. In this case, a male patient with stable angina and a moderate lesion by angiography in the mLAD (between the two arrows in Panel A and B). The lumen area measures less than 2.5 mm2 along the whole 25 mm of lesion length (Panel C to F), with an MLA of only 1.28 mm2 (Panel F), correlating with a positive iFR of 0.68. So, the lesion was treated with a stent implantation.
