Consultant update-Readable charts
I do quality of care reviews for several insurance carriers and if I had a buck for every chart I couldn't read because of illegibility, I'd have retired a long time ago. Please make your progress notes legible. You're setting yourself up for misinterpretation by consultants and, yes, your state board if the situation reaches that level. If you use a digital system them there's usually no problem. But if it's a handwritten entry, make sure everyone can understand your treatment. You're doing yourself an injustice if you don't.
