Our own Dr. Halima Ahmadi-Montecalvo, PhD, MPH took the stage with our partners at the NC Department of Health and Human Services and Sarasota Memorial Health Care System to share best practices for impactful health and social care network integration. For more than a decade, Unite Us has been at the heart of North Carolina’s social care infrastructure, powering the nation’s first-of-its-kind 1115 Medicaid waiver program, the Health Opportunities Pilot (HOP). Together, we’ve redefined what it means to provide care, addressing critical social needs for North Carolinians beyond the doctor’s office and delivering measurable cost savings.

Unite Us had a game-changing 2024, surpassing last year’s impact by nearly 60% and facilitating 24 million connections to care! Virtua Health Partners with Unite Us to Advance Population Health in New Jersey
Virtua Health is partnering with Unite Us to enhance care coordination by integrating its platform within Epic. This allows clinicians to seamlessly refer patients to essential services like food, housing, and mental health support while tracking outcomes. Through the Unite New Jersey network, Virtua aims to standardize referrals and improve patient well-being across its 3,000+ provider network.
