  • The Brooklyn Health Home?launched Community Care of Brooklyn (CCB) Navigator in collaboration with Unite Us. The CCB Navigator screens for patient needs and utilizes Unite Us’ coordinated care technology.
  • United Through Reading is a national CBO that serves military families by helping them stay connected while experiencing time apart.
  • Hire Heroes USA, a national CBO and veteran nonprofit organization, helps serve transitioning military members, veterans, and military spouses.
  • Military Family Advisory Network (MFAN) is a national nonprofit organization that identifies and bridges gaps in support of military-connected families.
  • The Giving Kitchen, a national CBO partner, provides emergency assistance to food-service workers through financial support and a network of community resources.?
  • The North Carolina Department of Public Safety (NCDPS) will use Unite Us’ technology to further improve reentry services for the formerly incarcerated.

  • The Connect Oregon network has expanded to Yamhill County with Yamhill CCO!
  • The Unite Oklahoma network has expanded with the Texas County Health Improvement Organization (CHIO)!
  • Our network in Utah has expanded to Carbon and Emery Counties!
  • The Connect Oregon network has expanded to Douglas County with Umpqua Health Alliance CCO and AllCare CCO!
