Comprehensive Care in High-Risk Obstetrics

?High-risk obstetrics is a crucial area of study and training for medical professionals, especially those specialising in obstetrics. This branch of medicine primarily deals with complicated and challenging cases during pregnancy and childbirth.

?Medical professionals get exposed to various high-risk pregnancy cases during their rotation postings as medical students, where we witness experienced doctors making quick decisions to ensure the best possible outcome for the health of both the mother and the baby.

?Practising obstetricians often find themselves working in emergency situations. Some have even experienced consecutive 48- to 72-hour shifts, witnessing both the joy of a healthy baby being born to a healthy mother and the sadness and panic that can arise from serious complications or even mortalities.

?Pregnancy brings about physiological changes that can lead to unpredictable complications, making it essential for obstetricians to remain vigilant and open-minded in order to identify and address potential causes that may result in severe complications.

?Even seemingly straightforward cases, such as a recorded increase in blood pressure during the latent phase of labour, may escalate into unforeseen complications during the delivery. An infusion of iron carboxymaltose, despite a test dose, could potentially lead to shock within minutes.

?Similarly, a normal delivery can sometimes result in postpartum haemorrhage, even when carefully monitored. Emergency cases, like a patient requiring an emergency caesarean section, can sometimes deteriorate into septic shock. The list of potential complications is extensive, and despite our training, we must be extremely vigilant and prepared to aggressively treat patients in order to save lives.

?Unfortunately, many obstetricians find themselves working in remote areas or in small setups with limited resources. In such situations, complications can create panic, and even well-trained professionals may find their thinking limited during emergencies.

?To overcome this fear and ensure optimal patient care, training in high-risk obstetrics has emerged. This specialised training focuses on enhancing our knowledge and decision-making skills through exposure to a variety of complex case scenarios.

?It covers topics such as managing eclampsia, Antepartum- postpartum haemorrhage, collapse, trauma, and the interpretation of the investigation required to diagnose the cause, critical care medicine dosages, and administration routes. Training also includes drills and simulations to boost our confidence in performing procedures like laryngoscopy for endotracheal intubation, cardiopulmonary resuscitation, and peripartum caesarean sections.
