Hi Asit, Nice article, I have encountered most of the above stated scenarios being part of transformation projects. I would like to add / elaborate one which may be closely related "Resistance to Change and Cultural Challenges". Being part of CGI most of the times Me and my team play a 'outsourcing partner' role to organizations. Two of the biggest challenges I have seen are 'lack of experience in outsourcing' and 'self preservation efforts by organizations' / Individuals. Both the above factors, limit our ability to influence, help organizations set right goals / vision / outcomes' out of these transformation programs. End up delivering bits and pieces of intended solution / vision. Hence delaying / not attaining the desired results. So Messaging and governance around transformation is one of the most critical success factors for me.

Leadership & governance is one area where we often see big challenges that are perceived to be hard to fix - may be due to authority, reputation and mindset. Leadership often lacks technology appreciation leading to ineffective handling of the resources (be it human or IT) and finances. One example is, in large transformation program, Tap is open with full throttle initially with the fancy terms like fail faster & early, learning for all, democratic decision making, unlimited access to resources etc.... but down the line surprisingly this will lead to reserves drained already even before a success patch is identified and defined. inexperience (leads to) -> ineffectiveness (leads to) -> favouritism (leads to) -> corrupt practices
