Common Cause

When we keep having problems, we need to look at the causes of our problems, mistakes, scraps or reworks. We need to look for what is called “Common Cause.” If multiple challenges seem to stem from the same root cause, we need to identify that cause and fix it. As an example, if we keep feeling a draft in the room, we might discover that the door is difficult to shut and stay shut. If we could fix that, we’d have far fewer people uncomfortable. If we keep overrunning our cost estimates, we might discover we did not allow for double checking our work. The old adage, “Measure twice, cut once,” comes from the realization that inaccurate measuring is the Common Cause of many avoidable errors.
