Call On Clare - Clare Cotter
We provide on-call Nurses and Support Workers who can be onsite within 2 hours if contacted during hour business hours which is 0900 - 1700hrs seven days a week.

About the role:
We are seeking a motivated and customer-focused Client Services Liaison to work as part of Call on Clare Scheduling team. You will report to the Lead Client Liaison Officer. You will undertake a range of administrative duties to provide assistance to new and existing brokered clients. Call on Clare works in partnership with our clients, medical professionals, and other allied health services to coordinate care based on respect, preserving dignity and choice, and assisting clients to maintain quality of life and independence in the community.
Responsibilities:
? Provide a welcoming and efficient initial interaction for potential clients by
responding to new enquiries and service requests and providing information
on our services.
? Act as the primary point of contact for current brokered clients, addressing
any inquiries, concerns or feedback that they may have
? Administrative duties involved with onboarding new brokered clients
? Intake of relevant forms and documentation and obtaining client information
? Consulting with clients and their families to assess care and support needs
? Prepare service plans and work collaboratively within the scheduling team to
allocate appropriate care workers for clients to provide continuity of care
? Regularly review and maintain client records to ensure all care is being
provided as per the care plan
? Liaising with relevant services and individuals involved in the client’s care
? Liaising with external organisations who are utilising Call on Clare’s brokerage
services, e.g. Eastern Palliative Care, Cabrini Hospital
About You:
Our ideal candidate will have experience in the health sector and strong

administrative skills. You will be an excellent communicator and display a high
degree of initiative, energy, and enthusiasm throughout communication with both
internal and external stakeholders. You will have impeccable time management skills and be able to manage competing demands.
Skills and Experience:
? Previous administration experience in a customer/client focused role
? Demonstrated experience in the healthcare industry is highly regarded
? Strong data entry skills, with thorough attention to detail and accuracy
? Excellent phone manner
? Excellent communication skills with a compassionate and empathetic
approach to your work
? Excellent interpersonal skills with the ability to use your initiative
? Highly organised with great time management skills
? Strong records management skills with a high degree of confidentiality
? Ability to work under pressure and manage multiple priorities
? An active team player and ability to work autonomously
? Have received 2 doses, plus a Booster of the Covid-19 vaccination
