Here is a list I compiled after a good deal of interaction with GPT-4 AI concerning statements by the critic mainly regarding the use of rotary NiTi versus 30o oscillating stainless steel relieved reamers, none of which are supported by the responses from GPT-4.

. Reamers cause more distortions and impact or extrude more debris than K-files

. Rotary NiTi cleans oval canals better than 30o oscillating stainless steel relieved reamers

. Rotary NiTi doesn’t produce micro-cracks in the dentinal walls.

. The balanced force technique cannot be improved upon.

. The balanced force technique cannot be mechanized.

. The mechanized form of the balanced force technique is not safer than rotary NiTi.

. Ultrasonic and laser powered irrigation systems don’t produce micro-cracks in the dentinal

walls

. The reason rotary NiTi is more popular than 30o oscillating relieved stainless steel reamers is

because it is superior.

. Sponsorship has no impact on education

. I have the ability to manipulate the responses from GPT-4

. What rotary NiTi cannot remove is compensated for by aggressive irrigation systems.

. Instrument separation is no longer a major concern when using rotary NiTi.

In addition here is mostly a list of relevant issues that the critic denies what GPT-4 validates.

. Sacrifices less dentin in canal preparation

. Reduces procedural stress

. Far more adaptable to the complete range of pulpal anatomy in terms of safety, efficiency and effectiveness

In short, 30o oscillations of stainless steel relieved reamers overcome all the downsides of rotary NiTi without introducing any negatives that would compromise its task of three-dimensional debridement with minimal trauma to the remaining tooth structure.

When the critic references studies that he characterizes as unbiased evidence-based data to support his contention of the superiority of rotary NiTi over 30o oscillating stainless steel relieved dreamers, he is referring to research that has been produced in an academic environment where 80% of the academic endodontists have been documented to be the recipients of industrial payments with the largest amounts, in the millions, going to those individuals that the corporations determine to be the most effective known opinion leaders. That combined with school administrators that determine what instrumentation system will be exclusively taught based on what company offers the most generous terms of sponsorship goes a long way in undermining the trust that would naturally result from a research and teaching environment free of corporate incentives. That is definitely not the case today and any claims of guaranteed impartiality are not supported by the obvious presence of corporate involvement.

From another perspective, the purpose of our hands-on endodontic workshops is to address and disprove the negatives one critic attributes to the 30o oscillating relieved reamers as well as giving the participants the opportunity to experience all the positives that this alternative method of endodontic instrumentation brings to the table.

One of the fundamental problems in our economy today is the influence financially powerful companies exercise in manipulating market choices in their direction. Competing on a dollar basis in determining which product gets the most exposure has little if anything to do with a critical analysis of the mechanical principles that support the systems. And, unfortunately, under the actual market tactics that prevail today, this fact that should be central to judgments is close to irrelevant. For sure, powerful corporations did not get that way by being passive. Hence, they attain their own advocates that attempt to diffuse anyone who proposes alternative means that just might appeal to a larger audience.

What I find most encouraging today is the introduction of AI algorithms that offer impartial insights and can evolve into highly sophisticated debates. I don’t believe the major corporations as well as the schools were counting on that, a vigorous source of an intelligent impartial and unbiased thinking process free of financial influence. These are conditions that to date are producing responses that are not bought and paid for by the industrial/academic complex. It will be interesting to see how this complex responds. For sure, it will respond to maintain their status quo advantages.

Many post graduates in endo look for future procedures that increase perfect treatment and prevent problems and redo for their patients. It's so important for great endodonists to get together and show that many procedures can lead to great prognosis.Dont get these great young future endontists get frustrated leading them to increase stress and look to other specialties.
