CenterWell Pharmacy is a critical link to improved health outcomes
CenterWell Pharmacy provides convenient, safe and reliable pharmacy services to millions of customers annually. Through our home delivery and over-the-counter fulfillment services, specialty pharmacy, and retail locations, we help customers manage their medications – at home or the location of their choice. We are even making it easier for customers to receive personalized care from our pharmacists and pharmacy professionals through digital experiences – like a new mobile app launched earlier this year that was designed with seniors in mind.
Medication adherence improves health outcomes
Nearly 80% of seniors live with two or more chronic conditions and 1 out of 3 seniors regularly take at least five prescriptions. Research suggests that chronic diseases, including type 2 diabetes, hypertension, and hyperlipemia, may contribute to increased risk of cognitive decline, Alzheimer’s Disease and dementia.
Medication adherence is crucial to disease prevention and management. A recent Humana Healthcare Research study revealed a correlation between medication adherence and newly diagnosed cognitive decline disorders. Compared to those who missed no adherence measures, risk for newly diagnosed cognitive decline increased as follows:
Nearly 7 million Americans are living with Alzheimer’s. By 2050, this number is projected to rise to nearly 13 million, and it’s currently the fifth leading cause of death among Americans 65 and older. These findings – based on comparing patients who missed medication adherence measures with those who met all these measures – highlight the importance of closely monitoring and promoting adherence. Importantly, the study itself shows a correlation between lack of adherence and cognitive decline, without confirming causality; in other words, it is unclear whether the lack of adherence causes the cognitive decline or if the lack of adherence is itself a marker of or caused by cognitive decline for other reasons. Effectively managing chronic diseases improves health outcomes and helps patients feel better every day, so they can live their lives and do what they love.

Our home-delivery pharmacy works with customers to obtain the lowest possible cost for maintenance and other prescriptions. Medication affordability remains top of mind for seniors, and we are committed to helping customers save on prescription drug costs. ?
Customers receive a 90-day medicine supply, reducing pharmacy visits, saving time, and controlling costs. The autofill program also simplifies the refill process, allowing for prescription refills via text, email, call, or app to ensure treatment plans are never interrupted.

5 个月The evidence is there. Pharmacist and nurse led interventions work. Often times, patients don’t even know what the medication does and how it works. A simple dialogue can go a long way. Asking the right questions, and navigating through the responses can absolutely improve outcomes with metrics to prove it.

5 个月Agree with the impact and criticality. This is really nice work at CenterWell but I also feel we need to take it to another level and drive wholistic care in the same fashion. There is no reason why such adherence information cannot be shared with primary health care providers and caregivers so that the plan can become action oriented and we can even become more helpful to our patients.
