Case Management Decoded

We’re excited to announce that we have case managers available to help families navigate the holiday season, so if you are struggling with addiction, please call us to ensure we can find the right treatment for you based on your individual circumstances — that also includes people who are suffering with codependency, those with substance use disorder who need help before or after seeking treatment, and for those who wish to recover without formal treatment.

If weren’t able to join our Facebook Live event last Sunday, we’ve pulled together a summary of our discussion about case management — what it is, and how it can help families recover from substance use disorder.

At a time when 21 million people suffer with addiction but less than 10 percent receive treatment, it is crucial that we do everything we can to improve access to quality care . Case management is beneficial to the whole family unit because it can lead to long-term recovery outcomes and a healthy family.

What Is Case Management?

Not everyone knows what case management is, or that you don’t have to suffer from addiction to be able to take advantage of the great benefits from this service.

The Case Management Society of America defines case management as a collaborative process that involves assessment, planning, coordination of care, evaluation, and advocacy. Essentially, case management is meant to provide options and services to meet an individual’s needs as well as those of their family. Working with a care team, a case manager can help identify appropriate providers and pathways of recovery while ensuring cost-effective solutions. The premise is founded on the belief that when an individual reaches their optimum level of wellness and functioning, everyone benefits — the individual, their family, and their support systems.

A case manager can be hired by the family or a spouse wanting to stage an intervention to guide their loved one to treatment, or an individual who wants to get help and support.

Case management isn't just for those suffering with substance use disorder , either. As we saw in last week’s Facebook Live, which featured Natalie’s story , case management can also benefit the family member who is struggling with codependency. Natalie suffered terribly with an addiction to her daughter to the point that it was making her critically ill. We were able to intervene and offer her free case management, ensuring that she gets the help she needs. We can’t care for our loved ones if we don’t care for ourselves first.

What Are the Benefits of Case Management?

While most treatment facilities discharge patients with follow-up instructions, sometimes their care ends there. That can sadly result in the person returning to using substances because they have no one to hold them accountable. They also lack the structure and security they had while in treatment.

The benefit of case management is that a patient can have someone work with them to offer further solutions to stay on track with their recovery. Case management is also an option for those who don’t wish to go into treatment at all . The aim is to find the right solution for the individual and to help facilitate their recovery.

The range of options that a case manager can offer is vast. They include:

  • harm reduction, should the person wish to reduce their using
  • an accountability coach
  • Clinical support
  • drug testing
  • Interventions
  • Wellness
  • Access to resources

Accountability sessions can be held in-person or online.

Our Goal With Case Management Is to Keep You Accountable

In the long run, case management can save money because it reduces the chance of relapse and the person can achieve their goals. It helps the family benefit from a healthy and functional recovery.

Something also worth noting is that open enrollment for health insurance is currently underway until January 1st, 2019. One feature that may be available is case management for substance use disorders. If you currently have a policy, contact your insurance provider to see if case management is currently covered.

Hey Ken , becoming an interventionist has been an ideal motto or title that I have been working on and thanks for the insight because it is most difficult to save lives when co-dependency is a factor or even when their is a family member that is causing more harm than good giving into them and losing them all in the same day as I have myself created an nonprofit organization. That deals with this kind of situation but the population I work with are actually homeless or homeless suffering from a co-occuring disorders while homeless and most are active gang members that are not kind to the intervention process because they already believe that family failed ?? them already because of their own ?? thinking pattern of the addiction itself and reaching out to these youths and even the elderly set in their ways are to be taken into consideration and carefully mapped out Continuum of care process in evaluation and assessment so any and all info that you present is all worth the time in reading literature and progressive change in patient standing as progress behind and addiction slowly begins to shrink ...
