CASE 6: Narrow middle vault, dorsal hump, wide/bulbous nasal tip

■ Minor Deformities

Narrow middle vault, dorsal hump, wide/bulbous nasal tip,septal deviation to left, mild tip under-projection, microgenia

■ History

This 19-year-old woman complained of nasal obstruction that was worse at night. She denied prior nasal trauma or previous nasal surgery. She also desired a more aesthetically appealing nose (Fig. 28.1A–H).

■ Analysis of Preoperative Photos

There is a dorsal hump, mild under-projection of nasal tip, and a wide, boxy nasal tip with “parentheses” deformity laterally, as well as internal and external nasal valve collapse.

■ Analysis of Postoperative Photos

The dorsal hump is improved. Under-projection and under- rotation of the tip have been corrected. The lateral crura of the lower lateral cartilages have been positioned more infe- riorly with a resulting improvement in nasal valve support.
