The purpose of this presentation is providing overview of the basic principles of care coordination, highlighting effective strategies for collaboration among healthcare professionals, patients and their families with the purpose of achieving desired outcomes. Nurses play a key role in the coordination and management of patients' care across healthcare continuum. They provide valuable insights into the needs of patients and ensure that resources are utilized optimally.? Nurses can help improve patient outcomes when they take a proactive approach to care coordination. One way nurses can ensure effective care coordination is by using evidence based practice. They must ensure that they stay up to date with the current care practices and standards, hence ensuring consistency in patient outcomes. Nurses should also advocate for patients to ensure that they receive quality care (Mackie et al., 2018). They can do so by advocating for additional resources and speaking up when the needs of the patients are not met. Further, as front line providers, they can educate the public on care coordination strategies.
