Improving California's health care system, so it works for all Californians.
关于我们
The California Health Care Foundation (CHCF) is dedicated to advancing meaningful, measurable improvements in the way the health care delivery system provides care to the people of California, particularly those with low incomes and those whose needs are not well served by the status quo. We work to ensure that people have access to the care they need, when they need it, at a price they can afford. CHCF informs policymakers and industry leaders, invests in ideas and innovations, and connects with changemakers to create a more responsive, patient-centered health care system. ?? Uncovering Hidden Truths in Health Data: The ANHPI Experience During the COVID-19 pandemic, initial data suggested the Asian, Native Hawaiian, and Pacific Islander (ANHPI) community was faring well. But Asian Health Services (AHS) in Oakland discovered a different reality. Key findings: ? Many ANHPI individuals avoided healthcare due to fear and policy threats ? Aggregated data masked disparities within subpopulations ? Disaggregating data revealed specific communities in need, like the Vietnamese population AHS's response: ? Reached out to community members ? Analyzed disaggregated data ? Established targeted pop-up clinics This experience highlights the critical importance of disaggregating health data to uncover hidden disparities and provide equitable care. Read more on our blog: https://lnkd.in/gzmmPjaH At CHCF, we're committed to improving healthcare for all Californians. ?? Convening on Partnering with Complex Power Dynamics ?? Thursday, November 21 9:30 AM - 4:00 PM Pacific In person in Oakland, CA In this CalAIM era, health care and social service organizations need to partner to address complex health and social needs through a more integrated and people-centered approach to care. These partnerships can often uncover power dynamics that limit effective collaboration and progress towards health equity goals if left unaddressed. During this convening, participants will discuss and address complex power dynamics, including exploring strategies for power sharing to build and maintain strong partnerships and developing skills for “power with,” like shared agreements, feedback loops, and accountability. Participants will leave the convening inspired to deepen their understanding of dynamics in existing and new partnerships, all in service of improving effective coordination across California’s health care delivery system to advance health outcomes and health equity. Leading the session will be two experts in quality improvement, health care partnerships, and power building:?Hunter Gatewood?and?AddieRose Mayer, MPH. The event is hosted by the California Improvement Network (CIN), a learning and action network sponsored by the California Health Care Foundation and managed by Healthforce Center at UCSF. This one-day convening is intended for those from California social service organizations, health care organizations, and health plans who are responsible for developing, managing, and maintaining collaborative partnerships to advance health. If possible, we highly encourage people to attend with colleagues from one or more of your active partnerships. Please note: We will prioritize one person per organization to fill the 80 spots available. If additional space becomes available, you may be able to bring additional team members. This convening is offered to participants free of charge. Travel stipends are available to those who need them. California has made remarkable strides in expanding health insurance coverage over the past 15 years. Here's what you need to know: ?? The uninsured rate has plummeted from nearly 25% in 2009 to less than 10% today. ?? As of 2024, California is one of only two states offering state-funded health insurance to all low-income undocumented immigrants. ?? While gaps remain, particularly for undocumented families above income thresholds, the state continues to push for broader coverage. ?? Despite these achievements, healthcare costs in California remain higher than the national average, presenting ongoing challenges. ?? While universal coverage hasn't been achieved as initially envisioned, the state now focuses on universal access to care. How can we further improve healthcare access and affordability? #HealthcareForAll #CaliforniaHealth #HealthEquity #AccessToCare

Providers working in home- and community-based settings can support person-centered care by meeting Medicaid members where they are. But transportation for field-based providers, such as doulas, community health workers, promotores, and behavioral health providers, is not directly covered by Medicaid, and travel costs present a challenge, particularly in reaching rural or geographically isolated areas. As California’s Medicaid program introduces new field-based services, providers face challenges adequately covering the costs associated with their travel, including mileage, public transportation, and unreimbursed travel time. This brief, developed with support from the California Health Care Foundation summarizes relevant Medicaid policies on reimbursement for provider transportation costs to deliver field-based services. It highlights approaches within California and across the country on how states and managed care entities can address these issues. https://bit.ly/3ARNUUH Logan Kelly
