A Blueprint for Implementing the ICHOM Stroke Standard Set
International Consortium for Health Outcomes Measurement (ICHOM)
ICHOM’s mission is to catalyze a global movement towards value-based healthcare.
The journey toward value-based healthcare is shaped by the ability to measure what truly matters—patient outcomes. In our recent webinar, Key Lessons from Estonia's Stroke Pilot: Process, Challenges, and Successes in Implementing the ICHOM Stroke Set, we explored how standardized outcomes measurement is transforming stroke care in Estonia and what the rest of the world can learn from it.
This engaging discussion featured leading experts; Terje Peetso , Chief Medical Innovation Officer at North Estonia Medical Centre, Vesa Komssi , Executive Vice President at Nordic Health Group , and Jennifer Bright , President and CEO of ICHOM.
Together, they took a deep dive into how the ICHOM Stroke Standard Set is being implemented, the challenges encountered, and the impact on patient care.
The Power of Standardized Outcomes Measurement
One of the most compelling takeaways from the discussion was the role of patient-reported outcomes (PROMs) in shaping care pathways. As one of the panelists highlighted:
"If we don’t measure outcomes that matter to patients, we can’t truly improve care. Data is powerful, but only if we use it to drive real change."
By collecting PROMs at 90 days and one year, Estonia’s stroke pilot is uncovering critical insights that help clinicians make more informed decisions, optimize treatment plans, and improve recovery experiences.
Lessons Learned: What Worked and What Could Have Been Done Differently
Implementing the Stroke Standard Set required seamless hospital coordination, clinician engagement, and data integration—and while there have been many successes, the panel also shared key lessons on overcoming obstacles.
One of the biggest challenges? Aligning stakeholders and ensuring long-term commitment to data collection and analysis. As one expert put it:

"It's not just about gathering data; it's about making it meaningful. Success comes when clinicians see how outcomes measurement leads to better decision-making and patient care."
