Learning about Unlocking Longevity through the Functional Movement Hierarchy

My goal was to review the learning around programming and frameworks for using equipment for a fitness or wellness lifestyle.

Hierarchy of Physical Function

The concept of physical function, particularly in aging, can be understood through a hierarchical framework that categorizes various aspects of an individual's physical capabilities and how they change over time. This hierarchy is crucial for designing interventions and exercise programs to maintain or improve functional abilities in older adults.

Recently, at the end of 2024, during the? #PPS show, talking to several PT owners and educators, one standard research article cited was?The 4-Element Movement System Model to Guide Physical Therapist Education, Practice, and Movement-Related Research.

Figure 1 shows the Movement System Diagram, which illustrates interactions among six physiological systems. While comprehensive, it can overwhelm clinicians and students. Guccione et al. ask: "What part of the body is excluded from the 'movement system'?" They argue that an alternative label for all body systems may not be beneficial. A coherent, widely applicable movement system model is currently lacking, hindering physical therapists from fully embracing their role as movement specialists. Levels of the Hierarchy of Physical Function

Another way of seeing this diagram is to see this as Levels of the Hierarchy of Physical Function.

1. Basic Physiological Capacities: This level includes fundamental physiological functions such as cardiovascular health, respiratory function, and metabolic efficiency. These capacities are foundational for all physical activities and directly influence higher levels of function[3].

2. Muscle Strength and Endurance: Muscle strength is the maximum force a muscle can exert in a single effort. In contrast, endurance is the ability of a muscle to perform repeated contractions over time. Both are critical for performing daily activities and preventing falls[3][5].

3. Flexibility and Balance: Flexibility involves the range of motion available at a joint, and balance is the ability to maintain the body's center of mass over its base of support. These components are essential for mobility and performing complex movements[3][5].

4. Functional Abilities: This level pertains to the capacity to perform activities of daily living (ADLs), such as dressing, bathing, and walking. Functional abilities are directly influenced by the underlying physical capacities[4][5].

5. Advanced Functional and Recreational Activities: At the top of the hierarchy are activities that require a combination of physical capacities, including sports and hobbies that older adults may engage in for enjoyment and social participation[5].

?The Disablement Process

The disablement process describes the progression from disease or injury to disability, highlighting the impact of physical impairments on an individual's functional abilities. This process is influenced by various factors, including age-related changes, lifestyle choices, and environmental factors[4][16].

1. Pathology: The initial occurrence of a disease or injury that can lead to impairments.

2. Impairments: Physical or mental deviations from normal functioning, such as reduced muscle strength or cognitive decline.

3. Functional Limitations: Restrictions in basic physical actions, such as walking or climbing stairs.

4. Disability: Difficulty or dependency in carrying out activities of daily living (ADLs) and social roles due to functional limitations[4][16].

Components of Physical Function

Physical function encompasses several components that are critical for maintaining independence and quality of life in older adults:

  • Cardiorespiratory Fitness is the efficiency of the heart, lungs, and vascular system in supplying oxygen to the body during sustained physical activity[3].
  • Muscular Strength and Endurance: The ability of muscles to generate force and sustain repeated contractions[3][5].
  • Flexibility is the range of motion available at a joint or group of joints[5].
  • Balance and Coordination are the ability to maintain postural stability and perform tasks requiring fine motor skills[5].

?

Functional Trajectory of Aging

The functional trajectory of aging describes the typical pattern of change in physical function over time. Initially, individuals may experience a gradual decline in physical capacities, leading to increased difficulty in performing ADLs. However, with appropriate interventions, such as regular physical activity and strength training, it is possible to slow the rate of decline or even improve functional abilities[1][2][3][5].

Exercise programs tailored to the needs and abilities of older adults can significantly impact the functional trajectory, promoting healthy aging and reducing the risk of disability. Key strategies include aerobic exercise, strength training, flexibility exercises, and balance training, all of which contribute to maintaining or enhancing physical function across the hierarchy[3][5].

If you haven't downloaded our screening booklet for practitioners, download it from this post below.

In conclusion, understanding the hierarchy of physical function and the disablement process is essential for developing effective strategies to support functional aging. Regular exercise and physical activity are pivotal in maintaining functional abilities and improving the quality of life for older adults.

Doing the work that is needed for change to happen.

I began my first newsletter by posing essential questions about strength training. For instance, what is strength training?

In this newsletter, I'm excited to share a valuable framework that can transform your approach to purchasing equipment for your gym or facility.

Real Value Equation

?

  1. Revenue vs. Utilization Gap - This framework reveals a crucial insight: members contribute an average of $58 monthly, yet a staggering $39 is lost due to underutilization. This staggering 67% inefficiency directly threatens your business sustainability and impacts member satisfaction. It’s time to close this gap for a healthier bottom line.
  2. Cost-Per-Visit Reality-With members averaging fewer than 5 visits each month, the effective cost per visit rises to more than $11.60. This high cost can lead to frustration and increased member attrition. Addressing this begins with understanding usage patterns and member engagement.

?

Business Implementation Strategies

?

1. Engagement Optimization?

???To tackle the utilization issue head-on, consider:

???- Implementing automated attendance tracking systems

???- Leveraging predictive analytics to understand visit patterns

???- Establishing early intervention protocols to re-engage declining members

?

2. Member Success Metrics?

???Focus on tracking these crucial performance indicators:

???- Frequency of visits

???- Average duration of each visit

???- Service utilization rates

???- Completion rates of programs

?

3. Clinical Integration for Physical Therapy Practices

Our data highlights valuable opportunities for physical therapy practices:

- Introduce hybrid membership models

- Create pathways from therapy to fitness

- Develop ongoing maintenance programs for patients post-rehabilitation

?

4. Outcome Measurement Framework

It's essential to track:

- Functional improvement scores

- Patient satisfaction ratings

- Adherence to treatment plans

- Long-term recovery success rates

?

Revenue Enhancement Strategies

?

1. Value Optimization?

???To minimize the monthly waste of $39:

???- Create tiered membership options that appeal to diverse needs

???- Implement usage-based pricing models?

???- Develop reward systems for consistent attendance to boost engagement

?

2. Retention Programs

???Concentrate on:

???- Establishing personal accountability partnerships

???- Developing effective progress-tracking systems

???- Creating recognition programs for achievements

???- Fostering community-building initiatives that enhance member bonds

?

Technology Integration

?

1. Digital Solutions?

???Utilize technology to:

???- Implement automated check-in systems

???- Analyze usage patterns for better engagement

???- Predict member attrition to address concerns proactively

???- Score member engagement to identify and act on at-risk members

?

Action Steps

1. Roll out robust tracking systems immediately

2. Establish baseline metrics to measure success

3. Develop targeted intervention protocols

4. Create engagement strategies that resonate with members

5. Continuously monitor improvement trends to adapt strategies

?

This value equation is not just a concept—it's a powerful tool that can help you bridge the gap between membership costs and actual utilization. By focusing on these key metrics and implementing targeted solutions, your business can thrive, leading to improved outcomes for the company and its members. Regularly monitoring and adjusting these metrics will result in streamlined operations and a stronger value proposition for your members.

?

Our goal is to convert the trend of underutilization into significant growth opportunities, enhancing member satisfaction along the way. This approach addresses immediate financial challenges and lays the groundwork for a sustainable model of success in both gym and physical therapy settings.
