Learning about Unlocking Longevity through the Functional Movement Hierarchy
My goal was to review the learning around programming and frameworks for using equipment for a fitness or wellness lifestyle.
Hierarchy of Physical Function
The concept of physical function, particularly in aging, can be understood through a hierarchical framework that categorizes various aspects of an individual's physical capabilities and how they change over time. This hierarchy is crucial for designing interventions and exercise programs to maintain or improve functional abilities in older adults.
Recently, at the end of 2024, during the? #PPS show, talking to several PT owners and educators, one standard research article cited was?The 4-Element Movement System Model to Guide Physical Therapist Education, Practice, and Movement-Related Research.
Another way of seeing this diagram is to see this as Levels of the Hierarchy of Physical Function.
1. Basic Physiological Capacities: This level includes fundamental physiological functions such as cardiovascular health, respiratory function, and metabolic efficiency. These capacities are foundational for all physical activities and directly influence higher levels of function[3].
2. Muscle Strength and Endurance: Muscle strength is the maximum force a muscle can exert in a single effort. In contrast, endurance is the ability of a muscle to perform repeated contractions over time. Both are critical for performing daily activities and preventing falls[3][5].
3. Flexibility and Balance: Flexibility involves the range of motion available at a joint, and balance is the ability to maintain the body's center of mass over its base of support. These components are essential for mobility and performing complex movements[3][5].
4. Functional Abilities: This level pertains to the capacity to perform activities of daily living (ADLs), such as dressing, bathing, and walking. Functional abilities are directly influenced by the underlying physical capacities[4][5].
5. Advanced Functional and Recreational Activities: At the top of the hierarchy are activities that require a combination of physical capacities, including sports and hobbies that older adults may engage in for enjoyment and social participation[5].
?The Disablement Process
The disablement process describes the progression from disease or injury to disability, highlighting the impact of physical impairments on an individual's functional abilities. This process is influenced by various factors, including age-related changes, lifestyle choices, and environmental factors[4][16].
1. Pathology: The initial occurrence of a disease or injury that can lead to impairments.
2. Impairments: Physical or mental deviations from normal functioning, such as reduced muscle strength or cognitive decline.
3. Functional Limitations: Restrictions in basic physical actions, such as walking or climbing stairs.
4. Disability: Difficulty or dependency in carrying out activities of daily living (ADLs) and social roles due to functional limitations[4][16].

Physical function encompasses several components that are critical for maintaining independence and quality of life in older adults:

Functional Trajectory of Aging
The functional trajectory of aging describes the typical pattern of change in physical function over time. Initially, individuals may experience a gradual decline in physical capacities, leading to increased difficulty in performing ADLs. However, with appropriate interventions, such as regular physical activity and strength training, it is possible to slow the rate of decline or even improve functional abilities[1][2][3][5].
Exercise programs tailored to the needs and abilities of older adults can significantly impact the functional trajectory, promoting healthy aging and reducing the risk of disability. Key strategies include aerobic exercise, strength training, flexibility exercises, and balance training, all of which contribute to maintaining or enhancing physical function across the hierarchy[3][5].
