This movie was presented by Dr Basma Darwish, MD, and awarded as Best Video of the 2nd SEUD Congress (Soc of Endometriosis and Uterine Diseases).
It presents our conservative approach in cases where deep endometriosis is responsible for multiple severe colorectal infiltrations and ureteral stenosis leading to hydronephrosis.
The surgery was carried out in a young nullipara presenting with hydronephrosis and painful digestive symptoms.
We performed:
1. the Rouen technique: large disc excision on low rectal nodule
2. Short segmental resection of sigmoid colon
3. ureterolysis
4. resection of LUS nodule along with 2cmsq of vagina
A majority of surgeons would probably have done ureteral resection and reimplantation into the bladder, and long low colorectal resection.
Instead of that, we preserved the ureter and 10 cm of healthy rectum and sigmoid colon.
16 months after the surgery, the patient is completely asymptomatic.
In our opinion, this conservative approach avoids unfavorable functional outcomes which could be due to an agressive surgery in deep endometriosis.
