AAIC Final Report on Pokhara Crash
Nepal's Aircraft Accident Investigation Commission's final report, released on December 28th, 2023, details the crash of a Yeti Airlines ATR 72-212A on January 15th, 2023, near Pokhara International Airport, Nepal. The crash, which killed all 72 on board, was primarily caused by both propellers inadvertently moving to the feathered position, leading to a loss of thrust and subsequent aerodynamic stall. Contributing factors include high workload, unfamiliarity with the new airport, and procedural errors under stress. The report highlights that despite the feathering, engines were operational, dismissing mechanical failure. It criticizes the crew's decision-making, lack of adherence to SOPs, and ineffective cockpit resource management. The AAIC recommends rigorous training, especially for new or challenging routes, and stricter enforcement of flight operation procedures to prevent similar incidents. The detailed analysis of flight data recorders and cockpit voice recorders provided insights into the critical moments before the crash, emphasizing the importance of comprehensive pre-flight planning and cockpit discipline.
Ice Safety Advisory Released
The Aircraft Owners and Pilots Association's Air Safety Institute (ASI) has introduced a new educational video as part of the Beyond Proficient: IFR series, focusing on mitigating airframe icing—a perilous condition causing significant risk, particularly for instrument-rated pilots. The video guides on preflight and in-flight strategies to avoid and escape icing conditions, emphasizing the importance of understanding weather patterns and forecasts. Airframe icing, responsible for approximately five accidents annually—almost half being fatal—can drastically reduce aircraft performance by altering aerodynamic properties, increasing weight, and impairing the function of critical flight surfaces. The initiative seeks to educate aviators on recognizing early signs of icing, implementing deicing procedures, and understanding the severe implications of icing on flight safety, including reduced control and engine failure risks. FAA Probes Bell 206 Emergency Landing
On December 28, 2023, at approximately 2:40 p.m., a Bell 206L-1 LongRanger II helicopter, registration N571CJ, owned by RotorPro LLC, encountered engine oil pressure issues and executed an emergency landing in a field near Walland, Tennessee, United States. The incident led to substantial damage to the aircraft, particularly a snapped tail boom, though all six occupants, including passengers and the pilot, reported no injuries. The Federal Aviation Administration (FAA) is currently investigating the engine failure and subsequent emergency landing of the 1983-manufactured aircraft to determine the cause and potential safety enhancements.
