To achieve success in value-based care, it is critical for a provider team to know the patients assigned under their umbrella of responsibility and the strategies to keep these patients aligned.

The first step in this process is for every patient to be assigned to an individual provider and to identify the costliest patients.

After patients are assigned, claims data is used to calculate key metrics for the provider's assigned population such as Acute Hospital Utilization (AHU), Average Risk Score, Diagnosis Recapture Rate and Financial Benchmark Performance.

These metrics provide a basis for how sick their assigned population is and quickly identifies performance improvement targets at a population and individual patient level.

The second step is to collaborate with the provider team on alignment strategies such as a patient campaign to connect with their assigned provider at least once per year for a comprehensive wellness exam or to engage in a chronic care medication management program that includes reconciliation and adherence processes via phone interventions each quarter.

Assign and Align is a key component for achieving quality and financial results.

Seems to be simple, right? I like how you put the emphasis on Assign and Align. More interesting to me was the example given after assign - the annual wellness visits with the provider and medication therapy management...I would have expected the provider visit but I am excited to see a pharmacy based patient campaign, instead of care management. Pharmacist have great relationships to so many patients and a wonderful resource for advancements in care.
