Although the basic procedures for anterior and posterior implants are similar, the latter is a bit easier to execute while it takes a great deal of experience and good clinical and anatomical knowledge to successfully insert an implant in the aesthetic zone.
There are certain additional challenges when it comes to anterior implant placement along with the conventional ones. Thickness of the tissue, getting good anchorage, color matching, smile designing, favorable tooth alignment and above all, patients’ expectations are to be taken in to consideration.
In order to achieve an optimal functional and esthetic outcome, the implant must be placed correctly in terms of the trajectory, adjacent vital anatomy, and positioning in all aspects viz. mesiodistal, buccolingual and apicocoronal.There are certain parameters which are to be set before we actually initate the surgery. The implant is planned in a position which is at least 2 mm from the buccal and lingual cortical plates, 2 mm from adjacent roots, 2 mm from vital structures, and 2–3 mm below the tissue crest. The angulation of the implant should be such that it should emerge lingual to the incisal edge of the planned restoration, which allows for a screw-retained crown.

If we come to patient’s assurance, it is important to understand the patient’s hopes and his expectations from the treatment. Natural tone and texture of the tissue should be noticed and great care should be taken to hide the metal line and black triangles at gum line. Of course, color matching is very important and we generally use ceramics as a choice of restorative material.
Anterior implant placement requires a lot of planning and can be intimidating at times, if you are in the early stages of the learning curve in implantology. This is a very recent case that Dr. Rajat has planned and executed successfully and the outcome was exceptionally good. The patient was 27 years of age and had lost his maxillary left central incisor to trauma. Trauma cases are very challenging to treat, especially when they are in the aesthetic zone. We started out by taking a complete intraoral scan using state of the art intraoral scanners available at our clinic. We then planned the entire surgery and decided to place a long 4.3 X 18mm NobelActive? implant in the area. This choice of the implant gives us good anchorage in the anterior area. We had engaged the implant with the nasal floor. During the surgery, we were careful to preserve the papilla and we also performed soft tissue grafting so that the gingival height is maintained and the final smile looks natural. A temporary crown was given immediately after the surgery so that the patient needn’t be toothless even for a day. During the subsequent visits, the healing was satisfactory and the tissues and implants were definitely on the right path.
By carefully selecting and diagnosing the case of anterior implants and practicing evidence-based customized treatment planning, and adhering to proven surgical principles, a predictable result is surely achieved.
