Q. What is outpatient in-home therapy?

A. Outpatient in-home therapy is also occasionally called concierge therapy services. This is a service where a physical, occupational, or speech therapist visits a patient who has an outpatient referral in their own home - essentially turning their home into the clinic space.?

This type of service can have many benefits, including providing the patient with solutions for problems they face with tools available in their home. It’s also much easier for a previously homebound patient to see a rehab professional in their home as they do not have to arrange transportation and then spend their entire afternoon going to and from rehab.

Additionally, outpatient in-home is covered by many insurers (including Medicare), can be covered by out-of-network insurance, or can be paid for with a reasonable flat fee. This differs from many other concierge medical services as it is designed to be accessible - and make physical, occupational, and speech therapy more accessible to a wider range of patients.

Q. What is the difference between home health and outpatient in-home therapy?

A. Outpatient in-home therapy is provided to those patients who are not homebound but prefer to receive therapy in their homes. Additionally, outpatient in-home therapy differs from home health services in how it is billed. No matter the label, all therapy is provided with the same quality and care.

Here is a breakdown of the differences between home health and outpatient in-home:?

Under Medicare guidelines, home healthcare is reserved for homebound patients requiring physical therapy (PT), occupational therapy (OT), and/or speech-language pathology (SLP) services, as prescribed by a doctor. Additionally, the patient must also have a plan of care signed off by their doctor. In general, any covered home health services are billed under Medicare Part A.

According to current Medicare rules for home health services, for a patient to be eligible to begin a home health plan, they must be:

  • Homebound;
  • In need of skilled yet intermittent nursing, PT, OT, or SLP care, and
  • Under the care of a physician.

In contrast, outpatient in-home therapy is reserved for patients who are not homebound but who prefer the convenience of receiving physical, occupational, or speech therapy in their home.

When it comes to Medicare patients, billing for these outpatient services falls under Medicare Part B, which can only be billed if the patient is not simultaneously receiving care under Part A. This applies even if the care being billed under Part A is only receiving nursing services.?
