Edith Wilson, often considered the first female U.S. president, though unofficially, played an extraordinary and unrecognised role during her husband Woodrow Wilson’s presidency after he suffered a debilitating stroke in October 1919. The stroke left him partially paralysed and mentally weakened, unable to fully perform his duties. Rather than allow the vice president or other officials to take over, Edith stepped in, shielding the severity of his condition from the public and Congress, and controlling access to him.

For the remainder of Wilson's presidency, Edith effectively made critical decisions, reviewed documents, and managed communications, though she referred to her role as "stewardship." She quietly assumed the responsibilities of the presidency, driven by the desire to protect her husband's legacy, particularly his vision for the League of Nations. However, her actions sparked controversy as she was unelected and unofficial in this position of power.

Despite her significant contributions, Edith Wilson received little recognition during her lifetime. At a time when women were still fighting for basic rights like suffrage, the idea of a First Lady taking on presidential duties was unprecedented and downplayed. Her legacy remains a subject of historical debate, with some seeing her as a pioneer of hidden leadership, while others view her actions as an overreach of authority. Nevertheless, Edith did not limit herself to traditional roles, stepping boldly into uncharted territory.
