As we reflect on the past 10 years, we are humbled by the ongoing trust of our practice partners, grateful for the commitment of our talented team, and eager to find more ways to accelerate the transformation to value-based care in the United States.??

Getting Started

Since launching Aledade in 2014 , Aledade has partnered with independent primary care physicians to help them form and join Accountable Care Organizations (ACO) in which doctors are paid to deliver the best care, not the most care.?We set out to empower doctors on the frontlines of medicine with cutting edge technology that helps them understand and improve the health of all their patients.?And we are doing just that.?Our purpose is the same today as it was when we began: to do what's good for patients, practices and society. Each of our partnerships today allows this important work to take hold in communities nationwide.?

Ten Years In

We started with a small group of doctors and practices in one accountable care organization and today serve more than 1,900 primary care practices and health centers in 45 states and the District of Columbia, with nearly 2.5 million patients covered by more than 200 value-based contracts across Medicare, Medicare Advantage, Medicaid and commercial payers.??

We are privileged to stand with and support primary care doctors to overcome challenges like COVID-19 and others which we could not have predicted when our journey began. We've stood shoulder to shoulder , and pushed forward in pursuit of our shared mission.?

Today, this work is improving patient outcomes - our partner practices conduct 37% more annual wellness visits than the average practice, focused on finding and preventing problems before they become emergencies, and in 2022 alone they prevented 46,000 hospitalizations and 81,000 emergency room visits. It means clinicians like Dr. Christine Meyer can use their shared savings to start a 24/7 emergency phone line for patients at her Pennsylvania practice. All of this is reducing unnecessary medical spend and making Medicare more sustainable.

This work is good for primary care, including physicians such as Dr. Tim Martindale , a doctor who achieved the financial security he needed to sustain his Texas practice and keep doors open.?And it's good for our society - reducing unnecessary medical spend and making Medicare more sustainable; enabling clinicians like Dr. Michael Maxwell , who led his Washington State community health center to national recognition for exemplary clinical outcomes.?These are some of the "whys" that continue to animate our work.

All of this means patients are receiving more coordinated care and improved outcomes, practices are succeeding, and our healthcare system is more sustainable. Our work to transform the healthcare system means improvements today and for the long term.??
