AI Execution Playbook: Why Most AI Strategies Fail & How to Fix It

Welcome back to AI for Business Leaders - Last week, we explored the AI Strategy Blueprint for Business Success, breaking down the key elements required to create an effective AI strategy.

So now that you have your strategy, selected your first AI initiative to support its delivery, and secured the investment, you're good to go—it's all plain sailing from here?

Unfortunately, not. This is where the hard work really starts, as many companies, even those with best-in-class strategies, struggle to turn AI plans into tangible business impact.

Why? Because execution is where most AI initiatives fail.

A BCG study found that while 90% of executives believe AI is critical to their company’s future, only 24% report meaningful business impact from their AI initiatives. That means three out of four AI projects fail to deliver results—surprisingly, not because the technology doesn’t work, but because businesses don’t execute effectively.

I seem to say it each week ??but as a reminder, AI isn’t plug-and-play. It doesn’t magically optimise workflows or generate value overnight. To get AI right, businesses must integrate it into daily operations, rethink outdated processes, and ensure employees trust and use AI-driven insights.

This playbook breaks down the four biggest execution pitfalls businesses face and provides a step-by-step framework for ensuring AI is successfully embedded into business operations, or drives process redesigns where needed.


1. AI is Plonked onto an Outdated Process

One of the biggest execution mistakes is treating AI like an add-on to legacy workflows. AI should either enhance existing processes or transform them altogether—not just be tacked on.

What Happens When AI Is Just Bolted On?

A global bank introduced an AI-powered loan approval system to speed up decision-making. Great, right? However, loan officers kept manually reviewing every application because the AI system didn’t integrate into their existing loan processing software. Employees didn’t trust AI recommendations because they couldn’t see how decisions were made, and the AI model was applied to a legacy process that hadn’t been optimised for automation, so approvals were no faster than before.

The result? Zero efficiency gains—and another AI system collecting dust.

Execution Fix:

1.?Conduct a thorough audit of existing workflows to identify inefficiencies, bottlenecks, and critical decision points.

2.?Redesign the process so AI streamlines repetitive tasks while ensuring human oversight remains where expert judgment adds the most value.

3.?Ensure AI seamlessly integrates into existing systems, providing clear, explainable insights that enhance—not replace—human decision-making.

4.?Pilot AI in one department before scaling and gathering feedback to fine-tune integration.


2. Employees Don’t Trust AI (So They Ignore It)

AI insights mean nothing if employees don’t believe in them. If AI contradicts instinct or experience, people will override AI recommendations—even if they’re more accurate.

What Happens When Employees Don’t Trust AI?

A large e-commerce retailer launched an AI-powered dynamic pricing tool to adjust prices based on demand, inventory, and competitor trends. However, category managers refused to adopt it because they didn’t understand why AI recommended specific price changes. The AI occasionally suggested steep discounts that didn’t align with seasonal pricing strategies. There was no way for humans to override AI suggestions, making them feel AI was “taking over” instead of assisting them.

Instead of driving better pricing decisions, AI was ignored—and the company went back to manual pricing.

?Execution Fix:

  1. AI should assist, not replace human judgment—blend AI recommendations with human expertise.
  2. Explain AI-driven decisions clearly with confidence scores, competitor insights, or historical trends.
  3. Create feedback loops so employees can flag and refine AI recommendations.
  4. Run A/B testing before full rollout—compare AI-driven vs human-driven decisions over months to prove AI’s value.


3. AI Relies on Bad Data (And Makes Bad Decisions)

AI is only as good as the data it’s trained on. If data is incomplete, biased, or outdated, AI will make flawed recommendations—which kills trust in the system.

?What Happens When AI Runs on Bad Data?

A major supermarket chain rolled out AI-powered demand forecasting to reduce stock shortages. However, within weeks, stores reported that shelves were either overstocked or empty. The issue? The AI was trained on pre-pandemic sales data, which didn’t reflect current shopping trends. The system ignored regional demand variations—it applied national sales trends to every store. It didn’t factor in supplier delays, so AI kept forecasting demand based on assumed inventory, not actual stock availability.

The result? Stock chaos, frustrated store managers, and lost revenue.

?Execution Fix:

  1. AI needs real-time, high-quality data—not outdated reports from two years ago.
  2. Automate data validation to detect inconsistencies, missing values, and biases before AI makes decisions
  3. Monitor for "data drift"—AI models must adapt dynamically if business conditions change.
  4. AI outputs should be reviewed against human expertise to identify where models need recalibration.


4. Leadership Loves AI, But Employees Aren’t Ready

AI isn’t just a technology change—it’s a people transformation. If employees aren’t trained or don’t understand how AI fits into their roles, execution will fail.

What Happens When Teams Aren’t Ready for AI?

A leading insurance company launched an AI system to detect fraudulent claims. Executives expected AI to reduce fraud losses by 30%, but claim analysts kept relying on manual checks because They weren’t trained on how AI flagged fraudulent claims. The AI model produced too many false positives, leading to extra work instead of reducing it. There was no process for handling AI-flagged claims, leaving employees confused about how to act on AI alerts.

The result? Fraud detection didn’t improve—and AI became more of a burden than a solution.

Execution Fix:

  1. AI should support workflows, not disrupt them—design AI-driven processes with input from the employees who will use them.
  2. Provide hands-on AI training before deployment, showing how AI insights fit into daily tasks.
  3. Allow employees to validate AI predictions and provide feedback to improve the system.
  4. Introduce AI gradually, starting with low-risk decisions before automating high-stakes processes.


Final Takeaway: AI Execution Determines Success

A well-crafted AI strategy isn’t enough. Execution determines whether AI succeeds or fails. The companies that get it right follow these principles:

  1. AI integrates into workflows—or drives process redesign where necessary.
  2. Employees trust AI recommendations—because AI is explainable, reliable, and improves over time.
  3. AI uses high-quality, real-time data—ensuring decisions are accurate and aligned with business objectives.
  4. Teams are trained and prepared—so AI adoption is smooth and effective.

Some useful observations Stephanie Gradwell so thanks for sharing. An additional, and key, observation I would make is that in integrating AI into their operational processes, and at scale, insurers have significantly underestimated the cultural adoption aspect of AI. Off the back of some consulting work last year, where more than 40 senior/exec level insurance professionals were surveyed, 72% said that their organisations have underestimated the size of cultural change required in effectively adopting AI. I'd also highlight the growing AI governance requirement for insurers in that the regulator is having an increasing interest in fair(er) outcomes being delivered to consumers and better catering for bias and ethics. That means insurers need to create greater levels of (AI model) explainability and auditability at a time where (AI) modelling complexity is increasing.

I couldn't agree more. This is the gap between businesses saying they use AI as a tick-box exercise compared to properly integrating it and driving value.
