90 Days In: Advancing CenterWell’s Integrated Healthcare Approach

90 Days In: Advancing CenterWell’s Integrated Healthcare Approach

It’s hard to believe I stepped into my role as President of CenterWell just three months ago. I’ve seen and learned so much in my short time here. From visiting CenterWell Home Health patients to traveling to our CenterWell Senior Primary Care centers, I have had the opportunity to see how we are advancing our integrated healthcare strategy. As a team – and a company – we share the same belief: integrated, value-based care is the future of healthcare. It’s what improves access, affordability, and health outcomes.

It has been a privilege spending time with members of our team across CenterWell Senior Primary Care, CenterWell Home Health and CenterWell Pharmacy.

I recently attended the 250th primary care center grand opening in Dallas, an exciting event for seniors and our amazing Dallas-Fort Worth market team. And, on another visit, I even had a patient challenge me to a game while spending time in our activity center – a reminder that our facilities are a bright spot in our communities and a place where our patients come for more than just care. We offer a place where people can build meaningful connections that support their whole-person health.

CenterWell continues to expand access to care across our communities – and our reach is remarkable when you look at the number of patients we serve through our care delivery capabilities:

  • Senior primary care centers serve nearly 266,000 patients in 12 states.
  • CenterWell Home Health cares for 350,000 patients each year.
  • CenterWell Pharmacy and CenterWell Specialty Pharmacy serve almost three million people.

CenterWell gives us a strong foundation to transform the future of healthcare. Through our home health, primary care and pharmacy offerings, we care for patients in the unique ways they want and need, in many settings. This is the core of what integrated care means – we’re providing comprehensive, value-based care from dedicated teams of physicians, nurses, pharmacists, social workers, and more.

Integrated care also means we take into account other factors that impact the whole person. Clinicians are not only thinking about our highest risk patients and the additional programs that might benefit them with respect to their medical care, but also social factors that affect their health and well-being, like food, medications, transportation and housing.

It's extremely important to us that through our integrated, differentiated senior care we’re able to deliver an experience and quality clinical outcomes that stand out.

In doing so, we continue to keep our patients at the center of every discussion about how we can make progress against our goal of a more integrated senior care experience.

Our entire CenterWell team is touching the lives of thousands of patients each day, many of them in underserved areas. Whether in their homes, in one of our clinics, or delivering a prescription to their doorstep – in many communities those needs would not be met if we were not there. I’m proud to work alongside the nearly 32,000 CenterWell employees who are making possible our ability to deliver quality, affordable care to those in need.

As we look to continue to integrate and scale our CenterWell offerings, I commit to keeping our patients at the center, always considering how we can improve navigating a complicated and fragmented healthcare system, so they can focus on what matters most: living and being their best.

I am thankful for this journey and look forward to continued progress improving the health of those we serve.

As a senior citizen I can appreciate your approach to Healthcare as it applies to senior citizens. However my experience with CenterWell thus far in 2024 leaves me with the opinion that we as seniors are more of a burden than that of a customer. Just one example. I was contacted by an account executive first of January very friendly and concerning. She set up my wellness visit. It was scheduled for 2/8/2024. It has been confirmed 4 times by CenterWell via phone and text. I received a call 2/6/2024 cancelling my appointment I was given a drop dead date of Monday afternoon 1-6 p.m.2/12/2024 and if not the next date would be late Feb or March. To my shock I received another text wishing to confirm the appointment on 2/6. The date of 2/12 does not work for me. I contacted Centerwell and was advised that it had been sent in error. Being a diabetic this appointment was very important as it would have allowed me to refill upcoming medications for diabetes and neuropathy issues. I may be reached at 770-364-0161 or [email protected] if you wish to discuss. If not you cannot say you were never made aware. Thank you for your attention to this correspondence. Mike Shelton

The transformational work you're doing at CenterWell is truly inspiring, Sanjay Shetty, MD. It's evident how a patient-centric, integrated healthcare approach can make a profound difference, particularly for seniors who often face multiple health and social challenges. The emphasis on treating the 'whole person' and addressing non-medical determinants of health like food, housing, and transportation is a vital paradigm shift in healthcare. Congratulations on the significant strides made in these early days at CenterWell. It's encouraging to see healthcare leadership focusing on quality, affordability, and holistic care. #IntegratedCare #HealthcareInnovation #HolisticHealth
