5-? What is the Kiloh-Nevine Syndrome?

What can we expect when our patient does not perform the #Pinch correctly? It may correspond to a #neuropathy of the anterior interosseous nerve (AIN), motor branch of the median nerve.

?? Patients with Kiloh-Nevine #Syndrome present #pain referred to the forearm or wrist, #paraesthesia or #paralysis of the Flexor Pollicis Longus and Flexor Digitorum of the Index and Middle finger, which explain the inability to pinch.

The most common site of compression of the AIN ? is the tendinous border of the deep head of the pronator teres muscle, followed by the Flexor Digitorum Superficialis arch, Gantzer′s muscle, thrombosis or fibrous bands.

??Although the #pharmacological treatment ??in patients with RA is essential to slow its progression and control symptoms, there is a significant percentage of patients who do not respond to the intervention or even suffer adverse effects.

#Exercise has been shown to be an effective and safe treatment, slowing the progression of the disease, controlling symptoms and being a potent #Anti-inflammatory (Katz P et al 2020). In 2015 the journal Nature Portfolio Reviews Rheumatology addressed in depth the role of exercise as a precursor of #Myokines ??that reduced the associated inflammatory processes (Benatti F et al 2015).

??And patients with worse pathological state and greater severity of #symptoms?

Not only has it been shown that exercise within a Multidisciplinary Approach should also be prescribed in these patients, but they ARE THE BEST RESPONSIVE TO #PHYSIOTHERAPY TREATMENT (Hagel S et al 2014).

7- ?? Low Back Pain and Empowerment

Nonspecific #LumbarPain ?? is characterised by being very prevalent in the population and presenting fluctuations and painful flare-ups over time.

It is therefore essential that the patient has de?? #tools that, as its name suggests, self-manage their symptoms. After all, we see the patient 1-2 times a week for 1 hour. ???El rest of the time will depend on their ability to #self-manage.

Education programmes, personalised therapeutic #exercise, monitoring of #symptoms, telematic control of the professional... are some of the tools that we can provide.

8-?? Stages of Frozen Shoulder? Fact or fiction?

Popularly, we know of 3 stages of evolution of #FrozenShoulder: inflammation??, Freezing ?? and Thawing?, progressing steadily to full recovery (Reeves B et al 1975).

However, this claim has recently been disproved?, along with established recovery patterns. A 2017 systematic review (Wong et al)?? analysed longitudinal studies to assess the natural history of the disease, finding that:

1? No study demonstrated a pattern of recovery consistent with the theoretical phases described above.

2? No work demonstrated complete improvements in ROM and function at the end of follow-up.

These results have recently been corroborated by Mertens MG et al 2023.

??Implications:

-Imperative that we educate patients and inform them of the need for personalised treatment.

-The assumption that it will resolve on its own is false and unrealistic.

-Delaying the start of treatment may leave sequelae.
