At Elements Support, we pride ourselves on offering a holistic and effective pathway to support young people. Our 7 Stages of Wellbeing Support guide every step of the journey, ensuring clarity, collaboration, and results. Each stage is designed to address the specific needs of students and young people, making a meaningful difference in their lives while empowering schools, homes, and organisations to achieve positive outcomes.

Stage 1: Referral – Taking the First Step

Making initial contact is simple and accessible. Schools, homes, local authorities, and organisations can reach out via:

Once we receive your enquiry, we determine whether the need is related to mentoring or group work for young people, or if support is required in the form of training, workshops, or keynotes. We clarify your goals and ensure we have the capacity and expertise to meet your specific requirements.

Stage 2: Needs Assessment and Engagement

If we agree we can fulfil the need, we arrange a Teams meeting to explore your requirements in more detail. This session includes a short, visually engaging slideshow about Elements Support—what we stand for, what we offer, and how we work collaboratively.

Through these discussions, we fine-tune our understanding of your needs and agree on the best fit from our three tailored packages—Elements Essentials, Plus, or Supreme.

Stage 3: Building a Tailored Package and Completing the Referral

Once a package has been selected, we begin the formal referral process. This includes:

  • Identifying the young people to be supported, discussing their individual needs, and confirming session arrangements (up to five students per day in a single school).
  • Assigning the right Elementor. Each Elementor is chosen based on their style, expertise, location, and experience, ensuring the best possible connection with the young person.

You will receive the Elementor’s personalised safer recruitment letterhead, along with their professional and personal profiles. These resources are shared with the young person to introduce them to their Elementor, building an immediate sense of connection and understanding.

Stage 4: Introduction at the School or Home

An in-person introduction meeting is arranged at the school or home, allowing the Elementor and the young person to meet in a familiar environment. This visit also provides an opportunity to understand the setting, meet key staff, and continue developing trust with everyone involved.

Stage 5: Crafting the Elements Solution-focused Action Plan

With the referral and consent process complete, and following the Elements Wellbeing Evaluation Scales, we create a solution-focused Elements Action Plan tailored to the young person’s unique needs. This plan is built with an Elementor Measurement Guide, which ensures the support process stays on track and outcomes are regularly measured.

All support work is centered around our 5 Elements of Well-being:

  • Patience – Taking time to understand and respond thoughtfully.
  • Presence – Being fully attentive and engaged with the young person’s needs.
  • Creativity – Offering innovative solutions for personal growth.
  • Curiosity – Seeking to understand the intricacies of each situation.
  • Authenticity – Building sincere and trustworthy connections.

These principles provide a strong foundation for all our mentoring relationships, ensuring personalised and effective support.

Stage 6: Monitoring Progress and Weekly Evaluations

Support is continuously monitored to ensure it remains impactful:

  • Every four weeks, the case is reviewed by the Elementor and either Steven or our Operations Director, Susanna. These reviews discuss challenges, celebrate successes, and explore possible adjustments to maximise benefit.
  • Regular input is welcomed from schools, homes, or organisations, alongside the views of the young person themselves.

We provide weekly evaluation updates, structured around the four elements of wellbeing:

  • Physical Fitness – Encouraging healthy body habits.
  • Mental Fitness – Developing emotional resilience.
  • Social Skills – Improving interaction and communication.
  • Environmental Awareness – Building a connection with the broader world.

These evaluations provide clear insights into the progress being made and allow you to stay fully informed.

Stage 7: Final Review and Transition

With four weeks remaining in the programme, we conduct a final review, exploring whether to transition to the end of support or move forward with a continuation plan. Decisions are made collaboratively, taking into account the needs of the young person while also considering practical factors such as budgets.

If support is concluding, we ensure a smooth and thorough transition with:

  • A transition plan, providing recommendations for ongoing SEMH (social, emotional, and mental health) support. This plan can be used by carers, teachers, or other staff to maintain progress.
  • A minimum of four sessions dedicated to transition, helping the young person adjust positively to the change.
