  5. Ask "Who": Identify the individuals or entities involved in the problem. Who is affected by the problem? Who are the key stakeholders or parties responsible for resolving it? Understanding the people or organizations involved can help in developing appropriate solutions.
  6. Ask "How": Explore the methods or approaches that can be used to tackle the problem. How can the problem be solved or mitigated? What actions can be taken to address the underlying causes and alleviate the symptoms?

Public are misguided and led in the directions Statisticians wants them to follow blindly, provide no room to think the reality of the situation or engineering model. I have not seen any materials talking about Engineering applications to detect problems and appropriate corrective actions to prevent the problem happening again. Industrial Statistics talks about Engineering applications in the manufacturing industry, a proactive approach system to define and control three systems in one they are: engineering, quality & manufacturing without any old data. Six Sigma advocates do not talk about the many failures of the Automotive vehicles recalls, medical devices recall, Product failures in the Aerospace Industries and in General Industries? Are you convinced that it is a totally foolish technology right in front of your eyes, kind of magic show. e. Why I am so strong on my comments? I was in your position for a long time attending several classes, seminars even taking Green Belt program, none of which did not answer my questions how to solve real time engineering problems in manufacturing industries facing. It is your money and your time, spend the time to learn something valuable to support the manufacturing industries.

Hi the issue is using why as a question to define a problem as you would want to use this question to solve a problem. It is better to drop the why question in problem definition I have studied this topic in great detail and have written papers on this topic
