Diptiman Chanda, formerly a researcher focused on the aging lung and idiopathic pulmonary fibrosis, now serves as a Senior Licensing Associate at the University of Alabama at Birmingham. Skilled Nursing Facilities Reducing Hospital Readmissions with INTERACT

Of the more than 1.6 million Americans currently living in nursing homes, approximately 60% are sent to emergency rooms and 25% are admitted to a hospital each year. Approximately 1 in 5 of these people are discharged from a hospital to a skilled nursing facility, yet are readmitted within 30 days. Joseph G. Ouslander, MD, a professor of geriatric medicine at Florida Atlantic University, developed Interventions to Reduce Acute Care Transfers (INTERACT), a program that is not only reducing hospital readmissions and thus patient complications, but is also saving Medicare billions of dollars.
