The 5-Pillars of HAIbridCare - Part 1

Thanks for reading NewHealthcare Insider with weekly strategic analysis and insight of the two transformations shaping healthcare: exponential technologies (AI, Genomics, Wearables), and Mobile/Retail/Home Healthcare Platforms.

Hello again friends and colleagues,

Happy Easter to all who celebrate it!

It was my intent to publish an evaluation of how I did with my Human-Centered Healthcare Platforms predictions that I published almost five years ago this summer. But a recent article about the future of healthcare being AI-Powered Platforms by Gianrico Farrugia, M.D. , CEO of the Mayo Clinic published by the Milken Institute provided a better setting for this retrospective. The concluding statement of his article leaves no ambiguity.

"This is not the future of health care—it is the present if we embrace the transformative power of AI and a platform model of health care."

My article published August 4, 2018 was short and to the point, so I'm including it below for those who might not have read it. It was titled "The 5-Dimensions of Human-Centered Healthcare Platforms: The Structure of Future Healthcare Organizations" [https://www.dhirubhai.net/pulse/5-dimensions-human-centered-healthcare-platforms-hchp-sam/]

We live in the age of platforms. Industry after industry is being transformed by the emergence of platforms: Facebook for social networking, Uber for transportation, Airbnb for hospitality, iOS and Android for smartphones, Amazon for e-commerce and many more. In a?recent article, I metaphorically proposed the emergence of a new type of competition in the healthcare industry. I was describing platform builders. It also outlined the risk posed by platforms to today's healthcare organizations. Today's article is meant to start the study of the evolving healthcare platforms based on the lessons learned from platforms in other industries applied to the specifics of healthcare.
This is a huge undertaking and definitely not one for a single person. I'm therefor making this and all future content on this subject available under the?Creative Commons license. I will also try to motivate the?Healthcare Innovation by Design network?(now over 15,000 global innovation leaders strong) to contribute to this undertaking.
I expect that this effort will progress through the following phases:
1. Improving and reaching consensus on the?DIMENSIONS?of HCHPs
2. Identifying the competency?ELEMENTS?of each Dimension
3. Defining the?PERFORMANCE LEVELS?of each Element
The effort would culminate in the creation of?COMPETENCY MATURITY MODEL?for HCHPs (HCHP-CMM) which would serve as a?BLUEPRINT?for Human-Centered Healthcare Platforms
Platforms are networks to which?Network Effects?apply. One or two networks will usually dominate an industry but many organizations can still thrive in a platform-dominated world. More platforms might be able to exist in the healthcare industry given its size. HCHP-CMM can also inform organizations trying to compete in a platform-world.
The 5-Dimensions of HCHPs:
HUMAN
Maximizing?HUMAN?health,?experiences,?relationships?and?decision making?(HERD) is the only measure of value creation in healthcare
Achieving?Maximum HERD?(MaxHERD) is the purpose of successful healthcare organizations
DATA
DATA?generated from the?ambient and continuous quantification?of HERD is the source of all future value creation in healthcare organizations
DATA?privacy and security?are paramount
TECHNOLOGY
DEEP LEARNING TECHNOLOGY?is the only available tool capable of processing the volume of DATA and generating the knowledge to inform every HERD event
CLOUD TECHNOLOGY?is the compute infrastructure for HCHP-scale organizations
ORGANIZATION
The purpose of HCHP organizations is to systematically explore the?HERD?FEASIBILITY-DESIRABILITY-VIABILITY?SPACE?to identify optimal healthcare service configurations which are necessary for achieving MaxHERD
AGILE MANAGEMENT?is the most efficient organizational structure for HCCPs and possesses an?insurmountable advantage?over bureaucratic hierarchies
DELIVERY
The?HUMAN HEALING RELATIONSHIP?will always be the cornerstone of healthcare delivery
Immediate, seamless?OMNI-CHANNEL ACCESS?(i.e., digital, retail, community, home and facility)?to healthcare services is critical to breakthrough outcomes
OUTCOMES-VALIDATED EXPERIENCE AND SPACE DESIGN?are integral to the creation of the healthcare services which are necessary for achieving MaxHERD

So let's see how I did.

Sadly, for the HUMAN dimension and using human experiences, relationships, and decision-making (HERD) satisfaction composite score as a measure of value-creation in healthcare, we haven't made much progress in my opinion.

On the DATA and TECHNOLOGY dimensions, I did reasonably well. When I proposed those dimensions, much of what I included sounded like science fiction when it came to healthcare. Very few organizations used cloud, remote monitoring was a novelty, and AI was believed to have no relevance because healthcare was just too complex! The last few months, put that opinion to rest.

On the ORGANIZATION dimension, it's a mixed result. Agile has definitely taken hold in some healthcare organizations but many more continue to use antiquated five-year budgets and plans which given the recent rate of change, I find pretty amusing. I am certain to the huge majority of organizations working on a five-year plan today, will not be around to execute year 5!

Finally, on the DELIVERY dimension, recent merger and acquisitions reflect that the digital/retail/home was a pretty good call. United/Optum, Walgreens and CVS are the farthest along on that path but others are also making moves in the space and I believe it is pretty clear that the future of healthcare is integrated healthcare experience regardless of delivery channel.

Overall, I would give myself 3/5 with my over-estimate on languishing of human-centeredness being the biggest driver of the misses. I am still optimistic, however.

I am strong believer of Abraham Lincoln's saying

So what should we create over the next five years! I believe the next evolutionary step in healthcare is HAIbridCare (pronounced hybrid care) Organizations with integrated Human values/AI capabilities as the foundation of a new healthcare model.

The 5-Pillars of HAIbridCare

  • Human Centeredness including both patients and providers
  • Deep Vertical Integration beyond care delivery
  • Proprietary Technology and Services
  • Global Scale
  • Unified Intervention Systems

I will explore the details of these five pillars in Part 2 next week.

AI in healthcare is sooo exciting, except of course for one small clinical problem: What Happens When Artificial Intelligence or ChatBots in Healthcare Collide with Insurer Prior Authorization Rationing? https://www.dhirubhai.net/pulse/prior-authorization-prevent-artificial-intelligence-ai-green-md/

Thank You Sam Basta. HealthCare has always been excelled with human values since many centuries, BUT it has been “One size fits all approach “ . However AI would truly transform healthcare to make it accessible, affordable and more personalised , precise and predictive. HealthCare with Human Values and AI capabilities will transform the way the health care is delivered.
