To mitigate the effects of human inconsistency and cognitive biases, healthcare quality systems must be designed with a deep understanding of human psychology. This means creating systems that account for human variability rather than punishing it. Hardwiring a ‘changed process’ is one such way that helps prevent slipping back to old ways.
