To continue our organization's important work of addressing long-standing health inequities, we are asking Congress, the administration, and regulatory agencies to:? ? - Pass the HELP Copays Act and ban copay accumulator and maximizer programs.? ? - Ensure telehealth services provided during the public health emergency are maintained and made permanent.?? ? - Mitigate the impact of restrictive utilization management practices on patients through prior authorization and step therapy policy solutions.?? ? - Enact regulatory or legislative solutions to abolish alternative funding programs.?? ? Today is National Healthcare Decision Day, which is held annually on April 16th. Do you know that there are times when you may need to make some healthcare decisions but not be able to? This day is designed to help you prepare for those times ahead of time. It’s a day when patients or healthcare receivers tell health professionals what kind of care they want to receive. Healthcare includes the maintenance or enhancement of health by preventing, diagnosing, treating, recovering, or curing illness, injury, or other physical and mental impairment. It’s provided by health professionals and the allied health field. Across the nation, healthcare facilities host National Health Decisions Day as the flagship venue for public engagement. The day is made up of independent events hosted across the country, with support from national media and a public education campaign. Founded in 2008 by Virginia healthcare lawyer Nathan Kottkamp, the goal is to make healthcare decision-making easier, faster, and more transparent for both the public and healthcare providers/facilities by providing simple, free, uniform tools that guide the process.

Healthcare is essential: everyone must have access to it. This is Health Equity Month, a time to focus on reducing the disparities that exist in healthcare access and outcomes. In honor of this month, we wanted to highlight our Community Paramedicine program. By extending care beyond traditional settings, community paramedics play a pivotal role in bridging healthcare gaps and fostering stronger, healthier communities. "Community paramedics can bridge the gap between medical need and accessibility, extending care where it is most needed. By reaching those in rural areas, the homebound, and the elderly, these programs not only provide vital medical services but also forge connections between underserved individuals and the support networks they require to survive." Hospital readmissions carry significant financial implications for healthcare providers and the system as a whole. ?????????????? Hospitals face increased costs from additional care and penalties under programs like Medicare's Hospital Readmissions Reduction Program (HRRP) for excessive readmissions, directly impacting revenue. High readmission rates also signal inefficiencies, leading to increased overall healthcare costs. By reducing preventable readmissions through better post-discharge care and patient education, hospitals can avoid penalties, cut costs, and improve patient outcomes. We all need to do our part in ensure proactive approaches are instilled in our policies!! ????????

I’ve always enjoyed a good political read - often it’s the challenge of trying to read the future ??? through the tea leaves of statements and suggestions. Some key thoughts and reflections from me as an NHS GP partner: ?? As ever an enormous shout out to primary care and colleagues working in the community - you are doing more, with less, despite rising complexity. ?? ?? It tells us what most on the frontline of care / patients would tell you - the current system doesn’t work for patients or staff ?? It tells us what most GP partners would tell you (if you can find them!) - you can’t squeeze more juice from the orange that is primary care - if you want more you need to invest more / differently ?? There is a laser focus on productivity - But, anyone keeping an eye on the autumn budget and response from the PM would be sensible enough to not see bucket loads of cash coming ?? Absolute necessity to focus on left shift of funding out to primary and community care and ‘locking’ this in but it will have to show outcomes easily and visibly to systems ?? Absolute need to focus on long term conditions as increasing rapidly with population and need to move more to prevention and management to reduce morbidity / mortality and associated cost ?? Technology should enable and drive productivity both in access and activity - the rest of society has sped away with innovation leaving us far behind in health. What does that mean for me as a GP partner today? ?? How do I manage Long term conditions? ICBs will be asked to focus on this in particular CVD ?? We need GPs doing what only they can given their reduced numbers ?? Multidisciplinary working is here to stay.? Rightly so - my practice is far more productive and offers higher quality of care given my team of physios, pharmacists, social prescribers and mental health workers. ?? Primary care remains an engine room of productivity despite the challenges. You are part of the solution, not the problem ? Data and outcomes will be key. No one likes a manual return on an excel but this is what will drive investment decisions at a national, system and local level. How can you show your data rapidly in those key discussions? ?? Estates - 20% of primary care estate predates 1948. My patients being seen in the portacabin on the patio attest to this. But given the capital spend required in hospitals we are unlikely to see new practices / expanded estate if at all. Partners need to consider how do you manage existing estate and maximize utility of your clinical rooms? A ticking time bomb or a very slow fuse that was lit back in 2012? The report highlights all the failings and the loss of productivity caused by mindless top down reorganisation of the service. But I don't read much on the back and now front door privatisation and wholesale contracting out of services. Huge contracts have been awarded to providers who now police the car parks, porter and clean hospital wards and manage the food and beverages side. That's before you get to outsourced waste and facilities management, equipment servicing and other aspects that are part and parcel of delivering our health service.

The recent findings from Lord Darzi's investigation into the NHS shine a powerful spotlight on an issue that has been intensifying for years; the declining mental health of its staff. The report outlines the immense pressures faced by healthcare workers, highlighting a system that is stretched to its limits. The significant rise of registrations seen at NHS Practitioner Health in the 4 years since the pandemic, as referenced in the report, is a sobering reminder of the toll that COVID-19 and the ongoing healthcare crisis have taken on the mental health of our frontline workers. The numbers speak for themselves; thousands of healthcare professionals are now reaching out for support: "It is my belief that there has been a very significant impact on the psychological wellbeing of NHS staff from the pandemic and its aftermath. NHS Practitioner Health was founded in 2008 to treat health and social care professionals with mental health and addiction problems. Since its inception, it has treated some 30,000 staff, amounting to some 20 per cent of the medical workforce that it covers." p.115 While it's encouraging that more staff are seeking help, this surge also highlights a deeper, systemic issue: the mental health of NHS staff has been neglected for far too long. These pressures are not only affecting patient care but are taking a serious toll on the mental wellbeing of those who work tirelessly to keep the NHS on its feet. Key Findings on Staff Mental Health: - Alarming rates of burnout, depression, and anxiety among NHS staff. - Increased absenteeism and staff turnover linked to mental health struggles. These findings are deeply concerning but not surprising. Healthcare staff, particularly in recent years, have faced unprecedented challenges. The strain of long hours, increasing patient numbers, and resource shortages has left many feeling overwhelmed and unsupported. The rising registrations are not just numbers; they represent the human cost of a system under pressure. Lord Darzi's report is a critical reminder that investing in our NHS should also mean investing in the people who are its backbone. If we are to safeguard the future of our NHS, we must take immediate and meaningful steps to protect the wellbeing of those who care for us.
