Grief and loss are a personal and sometimes confusing experience. Most people are familiar with the 5-stages of grief as described by Elizabeth Kübler-Ross, but did you know this has since evolved to 7-stages? The 7-stage model is widely accepted as more inclusive and accurate to what people experience. Physicians devote their careers to improving and preserving their patients’ health and longevity. But along with the success and fulfillment physicians often derive from practicing medicine, grief and loss permeate the profession as well. In the traditional culture of medicine, the latter emotions are not just undiscussed, but also poorly understood. We're enjoying a wide-ranging panel discussion today at #HCCT24 with Adam Ackerman of EDCi, Candace Dietrich of Carson Tahoe Health and Jacqueline Pilon at State University of New York Upstate Medical University. Some snippets: "The #contactcenter isn't just a group of people in the corner doing something," Candace says. "It's critical for the success of your patients to build relationships with different departments in your team, especially IT." "Time is a perception," Jacqueline says. "Every second feels like a minute, especially for your nurses in a time of crisis. That's why sharing reports with your team is so important because you can show them what's actually happening; not their perception of what's happening."

I would argue that ensuring Community Health Workers and other staff with expertise (and time) related to addressing health-related social issues are critical to care coordination efforts. So often it is not anything clinical impacting length of stay, readmissions, or avoidable ED utilization. #valuebasedcare #caremanagement #carecoordination #SDoH #HRSN

Today is a good Monday. We have 18 open spots out of 24 for patients to be seen today. As we continue to improve I wanted to share our latest progress here as we work to improve how we provide primary care to our patients. For those of you who have been following along our team has made a significant and concerted effort to improve our access and the experience of care for our patients. We currently serve over 2000 patients and families in our practice and over the last 6 week cycle we have made a concerted effort to make our office more easy to access, a calmer workplace for our employees and a place where we effectively team together. What did we do? - install a doorbell for patient check ins to limit interruptions for our secretary - work as a team to triage all patient concerns rather than delegate this to a single team member - emphasize that prebooked appointments are for care that can only be received in person (well baby visits, chronic disease, antenatal visits) - work to optimize all visits before they are seen in clinic: this means lab work or imaging done before we see patients in some cases to ensure we can optimize our time in person - instill the principle of acknowledging receipt of requests vs feeling pressure to complete them the moment they are received - emphasizing to our team that their time and health is critical to our success and that being calm is more important than having an inbox at zero. - we have opened nearly every day to be fully accessible by online booking and patients are able to book as it works for them. Do I anticipate some hiccups into the future - it's likely. #Fridayreflection Do you add your milk first or after your have poured your tea? Another busy week but continue to be amazed by my colleagues who are our frontline in the #NHS as well as those providing leadership. And many thought provoking conversations regarding education and training as well as a reset and reform of what we do every day. Had a great conversation around outpatients and how we measure what we do. So much has changed over the past few years: how we interact with patients, how we work with colleagues across primary and secondary care, how we follow up and share decision making of treatment options but we have not kept up with measuring all these changes. Do you remember watching Top of the Pops or listening to the announcement of the week's No 1 pop song when you were younger? The number 1 song was that which had sold the most singles that week. Now, it depends on downloads, sales of hard copies and streaming activity. This is how outpatients has changed with virtual, telephone, written and face to face appointments! Outpatient activity measures now need to become more holistic. Not just activity numbers or new to follow up ratios. How do we measure patient centric outpatient activity that delivers what patients wish for, in a way they want and where this works for them? And if we are changing how we deliver our outpatient activity, how are we changing our training offers so that we teach how we triage, decide on straight to test or a one stop clinic? How do we ensure that every single consultation does include shared decision making? Massachusetts Health Policy Commission’s Kate Scarborough Mills presented on health care market consolidation at #NASHPconf24 yesterday. It made me realize that it’s been over 10 years since Massachusetts passed Chapter 224 health reform legislation. Yikes! Where are we now with Chapter 224? Former ch.224 CHIA team Alex Lai, Dorothy Zirkle , Caitlin Sullivan, Nathan Bosdet, Kait O'Brien, Elizabeth Arnold - what do you think? (Miss you all! eReunion?!) Thoughts anyone else? We still can’t clearly determine how much market power drives some hospitals to be higher cost versus other factors like some hospitals providing specialty care, research, and cheerier, art-filled facilities. Surgeries and other services subsidize maternity care like commercial payments balance out lower public payments. Try asking a hospital for a list of costs for how much a stay for heart failure costs versus having a baby. And hospital charges do NOT equal hospital cost. Where do we go from here?
