It’s been a few weeks since speaking with Mike Stack on his U of Michigan Wellness Paradox podcast. Since then I have had the pleasure of meeting new (& hearing from current) clinical & non-clinical professionals re: the Podcast/ Career, Healthcare, Well-being, Wellness & Lifestyle. In follow-up, a prominent MD recently suggested providers should want to share their health metrics with patients as a positive way of highlighting how they live their day is what they practice w/ their respective patients/clients. It’s a reasonable ask & to that extent, metabolic health is key to better healthspan (quality of life) & potentially lifespan (length of life). One lab that brings significant value to a comprehensive metabolic review is Insulin. Fasting insulin acts as a great early detection system of future chronic diseases such as T2D, CVD, Cancer, and Neuro-Degen. Despite its value with optimizing healthspan, fasting insulin is not currently a common measure of metabolic health in primary care. However, I am optimistic this lab among many more will become standard of care as momentum for preventative care and wellness continues to increase in the US! As a Sr. Healthcare Leader, Lifestyle Medicine Advocate & Practitioner who went through a 180 turnaround w/ their health 3.5yrs ago, I have always been focused on the wellbeing of the community be it bringing new services, building new facilities or more recently guiding community members to better health! In full transparency, below is my recent insulin lab result along with a major Lab provider such as Quest provides as reference range for high risk to optimal (low) risk where <18.4 uIU/mL is optimal. Of note, some experts focused in metabolic health suggest <5-7 uIU/mL is optimal for fasting insulin. In addition to my Insulin results, my fasting glucose was 86 and A1C was 5.2. This was my first insulin test and I’m very grateful for the partnership I have created with my PCP. He and I have great discussions and I’d we have learned from each other as well! Through my recent personal and professional journey, my purpose, mission, and passion as a Sr. Healthcare Executive, Practitioner, and Advocate has improved and transformed me as a person and as a leader. I’ve long made it my mission to effectively change how healthcare is delivered, one patient at a time. Men are 33% less likely to go to the doctor than women. In fact, more than 40% of men don’t go to the doctor at all unless they’re experiencing a major health issue. Studies suggest that men traditionally rely on their partners for many health decisions. But this avoidance of health care comes with a steep cost. For example, deaths from melanoma are 50% higher in men than women, despite a 50% lower incidence of the disease. Engaging men in their health holds the potential to deliver massive improvements in health outcomes, costs, and quality of life. Cleveland’s University Hospitals (UH) resolved to address this challenge through the creation of the UH Cutler Center for Men. This visionary men’s health experience aims to address men’s unique needs and invite them to be leaders of their own health care journey. For at least 25 years there has been sufficient scientific proof that physical activity is extremely beneficial to health and well-being, both generally and in many disease-specific scenarios. So why doesn’t our health system cover exercise management services? Society A relies on highly bureaucratic, centralized health care systems. Professionals certified in lifestyle and social interventions address SDoH and unhealthy behaviors, using complex tools and billing processes in a highly regulated, expensive and cumbersome EMR designed to serve the system (not the users). Medical malpractice attorneys advertise on TV for people who have experienced medical neglect. The system prioritizes "Big Data" and technology, empowers large corporations and turns health care professionals into overburdened millworkers that form unions. Patients bear the cost of these health promotion services as “personal accountability”, paying out-of-pocket with limited tax deductions and sometimes being able to use health savings accounts. Society B fosters a decentralized, community-driven approach. Businesses compete to provide lifestyle and social intervention services, empowering consumers to choose what works best for them. These businesses being lightly regulated, tort attorneys advertise through personal injury ads on TV for people who are harmed. Instead of rigid billing codes and complex data systems, whole health software tools support local community collaboration and innovation. Society B values health as a collective priority, recognizing that healthy cultures produce more value and wealth, providing tax- or employer-supported discounts through a “Stay Well” plan. This plan provides everyone access to a limited number of discounted services annually, making wellness affordable and prioritizing prevention over treatment for better, longer lives. We live in Society A. With that bias, Society B looks tempting (their grass looking greener), but I doubt Society B would end up fully satisfied - according to T.R. Reid, no society in the world is fully satisfied. The notion of charlatans restrained by tort is un-nerving, but – closing in on 40 years as an invested observer – I’m underwhelmed by the power of board certification and regulation to deliver cost-effectiveness and high quality. Certification primarily protects “turf” (aka scope-of-practice), regulations marginally increase safety but stifle creativity and drive up cost. Free markets foster innovation, but ill-informed and unwitting consumers can get swindled. Societies need a mix of free markets and regulations. After decades of stifling the promotion of health and well-being services, I think it's high time that we take a look at some of what Society B has to offer. A recent Fast Company article highlights a critical issue: the traditional clinical trial model often fails to serve diverse populations, impacting health equity. Citing findings from #USCSchaeffer Center’s “Future Elderly Model,” the piece notes that hundreds of billions of dollars could be lost over the next 25 years due to reduced life expectancy, shorter disability-free lives, and fewer working years among those underrepresented in clinical trials.
