As we continue to stive to provide better care at lower costs, we have to think outside of our organization and work together with our community partners like primary care providers. ACO Primary Care Flex model?starts January 1, 2025, and aims to increase primary care provider (PCP) participation in ACOs and the Shared Savings Program, as well as to increase the number of people with Medicare in an accountable care relationship.
