How Emergency Departments Play a Role in Hospital Readmissions The emergency department (ED) plays a critical role in hospital readmissions, often serving as the initial contact point for patients experiencing complications post-discharge. Dexur’s data reveals that many readmissions within 30 days originate from the ED, highlighting gaps in continuity of care such as inadequate follow-up, poor patient education, and insufficient outpatient support. Hospitals are adopting innovative strategies to address this issue, including Hunterdon Health's system for arranging next-day appointments and home monitoring for patients visiting the ED post-discharge. Dexur's advanced analytics platform provides detailed data on ED-related readmissions, both within the same hospital and across other facilities, enabling healthcare providers to identify patterns, understand patient cohorts, and implement targeted strategies to reduce readmission rates. https://lnkd.in/gT56JaFX #HospitalReadmissions #EmergencyDepartment #HealthcareAnalytics #PatientCare #PostDischargeCare #PatientEducation #OutpatientSupport #HealthcareInnovation #ReadmissionReduction #ContinuityOfCare #HealthcareData #EDReadmissions

One of the salient points from this article was that digital transformation and innovation were seen as needed for quality care, recognising the importance of flexibility to respond to patient preferences and needs. Without this, digital tools could act as a barrier to good quality care. Digital tools such as patient portals and patient-facing apps can support more seamless communications and better access to care. #digitalhealthinnovation #digitalhealthcare #wayforward #bridgethegap #thepatientsassociation #outpatientsstrategy
We’ve been working with NHS England and the Royal College of Physicians on a new strategy for outpatient services. In the first of three blogs, we reflect on what we've heard from patients about their experiences of outpatients. In the other two blogs we look at staff education, health inequalities, integrating care, staffing, patient choice, personalised care and treating the whole person. https://ow.ly/cNk650QszpP

The latest findings from the SafeCare Study reveal a startling reality: 7% of patients experience an adverse event during outpatient care, with 63.8% of these incidents being drug-related. This study, led by Mass General Brigham and analyzed across Massachusetts outpatient practices, underscores the urgent need for focused #PatientSafety measures beyond the hospital walls. Healthcare leaders must prioritize and expand our safety protocols to cover all care settings, ensuring every patient encounter is as safe as possible. It’s time to bridge the gap between inpatient and outpatient safety standards. #HealthcareQuality #OutpatientCare

?? New Evidence Brief: What works to address inequalities in the Primary-Secondary care outpatient interface ?? 15 million GP appointments lost every year because of issues between primary and secondary care. Some people are better at navigating the referral process to get the care they need than others - leading to inequalities. ?? Here we highlight six points at which inequalities arise in the primary care secondary care interface and propose evidence-informed recommendations. 1. Referrals from primary care 2. Navigating the system 3. Secondary care appointment 4. Specialist follow-up 5. Patients who do not attend 6. Self referral #healthinequalities #equitableNHS https://lnkd.in/e_6gVdXu Amy Dehn Lunn Bilal A. Jenny Blythe Serge Engamba Payam Torabi

"28 rural communities lost access to inpatient care last year as a result of rural hospital closures or conversion to a model excluding inpatient care." At PHG, we are working feverishly to stop closures, expand quality of care and access to care - while bringing strategies and efficiencies to the operational side. Healthcare is not getting easier, the needs and demands of rural operators is changing - and a better mouse trap needs to be build to support these healthcare consumers and overall communities. https://lnkd.in/d3DdNHW9

Safety issues are common in primary and ambulatory care. Estimates indicate that 20%-25% of the general population experience harm in this setting. Around 4 out of 10 patients are harmed. Most of these errors are related to diagnosis and medication prescription. Up to 80% of this harm is preventable. 1. Auraaen, A.,?L. Slawomirski?and?N. Klazinga (2018), "The economics of patient safety in primary and ambulatory care:?Flying blind",?OECD Health Working Papers, No. 106, OECD Publishing, Paris. 2. Sarkar U, Bonacum D, Strull W, et al. Top thing on my mind this afternoon: Our area is rural. It feels like most of us grew up about 10 miles away from the nearest hospital. We also had to walk uphill to and from school but that is a different story. A new federal program, which went into effect in 2023, could potential save rural hospitals from being shut down. The catch? "The Rural Emergency Hospital program guarantees hospitals extra cash if they provide emergency and outpatient services but end inpatient care." This article is a in-depth observation of the program and the merits - and downfalls - it could provide for rural hospitals. https://lnkd.in/gu8BpTN8
