Physiotherapists and Speech and Language Therapists are not taught how to treat facial nerve palsy as part of their degree at university. The muscles in the face are different to elsewhere in the body. Your face does not need to carry the shopping in from the car. Your face does not need to lift weights at the gym. Instead the human face moves gently and delicately, intricate facial expressions helping us to communicate with others. Some people have bilateral facial palsy and are unable to move their faces in the expected way. They may rely heavily on body language and intonation to show how they are feeling. With Bell's palsy, Ramsay Hunt syndrome and some other causes of facial palsy, there will be different degrees of recovery to the face. If these patients see a therapist without the correct training, they may be given gross strengthening exercises which are more suitable for other parts of the body. This can lead to muscles in the face becoming overactive. This is why it's important to see a Specialist Facial Therapist for disorders of the facial nerve. Unfortunately many health professionals are unaware of this and also there is a lack of funding in the NHS for specialist facial therapy. Why am I talking specifically about the nerve? The other type of facial palsy is caused by a stroke. A stroke differs in that messages from the brain do not reach certain parts of the face, causing the face to droop. This is a central cause. With facial palsy that involves the facial nerve, there is inflammation around the nerve or damage to the nerve itself. With facial palsy due to a stroke, in most cases the forehead is spared, so the patient will have facial droop but still be able to raise both eyebrows. Where the problem is with the facial nerve itself then the patient will usually be unable to raise the eyebrow on the affected side. With a stroke they will also look for other symptoms as well such as limb weakness etc. Facial palsy due to a stroke is different to facial palsy caused by a problem with the facial nerve. Not all facial palsies are equal. Facial palsy is complex but unfortunately does not get much recognition and NHS pathways of care are mostly poor or non-existent.
