Technology is one of our key differentiators because data always tells a story. Our proactive focus on performance, compliance, and personalized resident care is essential to our success. We track clinical outcomes rigorously in our weekly multidisciplinary team meetings, which include nurse practitioners and our rehab directors from EmpowerMe Wellness. We monitor ER visits, documentation rates, hospitalizations, falls, medication errors—you name it. This data-driven approach allows us to identify trends and make informed decisions to enhance resident care. In just one month of implementing AllieHealth across our portfolio, we saw a remarkable 14.1% increase in Solera’s documentation rate. At several of our communities, we've also achieved a reduction in hospitalizations, thanks to partnership with CarePredict, Inc. By flagging team members about residents at risk for falls and UTIs, we've been able to intervene early and prevent serious health issues.
