Bus and coach operating systems are often complex and require people with the right skills, specialised tools, and knowledge to effectively carry out repairs and maintenance to help ensure they operate safely. A new OTSI investigation report into a 2022 coach rollaway and collision in the NSW snowfields highlights how unlicensed maintenance and repair work can lead to adverse safety outcomes.? The incident happened on 5 September 2022 when a coach was transporting 36 children and 5 adults from Smiggin Holes following a ski lesson as part of an excursion to Kosciuszko National Park. During the trip, the coach lost its brakes and rolled backwards down Kosciuszko Road, with the driver forced to swerve to avoid a collision with oncoming traffic. The coach reached speeds of up to 105 km/hr before the driver decided to veer the coach off the road where it travelled down an embankment, struck a rock outcrop, and came to a stop.?? Multiple children and adults were transferred to hospital for observation with some passengers suffering minor injuries. The coach sustained damage to the rear of the vehicle.? ??The investigation found:? - Unlicensed and non-compliant maintenance work was a main contributing factor in the failure of the coach’s braking system.? - The coach had several defects that did not comply with the requirements of the heavy vehicle standard of the Heavy Vehicle National Law (HVNL).? - There were several safety factors including air brake system faults, disabled driver alerts, and a lack of driver familiarity with the vehicle, its system alerts and recent maintenance history.? ? ?? OTSI has identified several key considerations for bus operators and owners to prevent a similar incident from occurring: - Operators must ensure that their vehicles are maintained by licensed mechanics in accordance with original equipment manufacturer (OEM) and National Heavy Vehicle Inspection Manual (NHVIM) requirements.? - Operators and drivers should understand the importance of alarm displays, as cues to indicate that there is an issue with a safety critical system and the correct required response.? - Pre-departure checks and driver handover procedures provide opportunities to identify system faults and should be a standard practice included in the operator's safety management system. Anybody with 5 years of experience doing a pretty narrow thing is an expert, right? Like, driving. Expert! And, yet, accidents occur. People are gonna click the links. Some of them some of the time. All of them at least once. AND - we need training on how to not click the links most of the time. It's not either/or. We need that training for our computer use at work AND at home and to share it with the people we love. We need technology to safety net us and return us back to the cars after we drive offa the link cliffs. We equip new drivers with driving training. Well, people, that information superhighway has some of my mayhemmy black cats driving along it. There are gonna be accidents. Aaron Birnbaum gave me the idea for this one. I've always wanted a Toonces-themed post anyway. Drive and click intentionally. Most of the time.

Urgent Information for Fleet Operators regarding the Transport for London Progressive Safe System and Vehicle Operators responsibility. TfL have confirmed that a 6 month grace period will apply to the October 2024 introduction of PSP. It remains a fact however that a significant number of systems currently being sold and installed with supplier claims of compliance are in fact not compliant. TfL statement "The DVS operator’s guidance sets out that equipment fitted to a vehicle as part of the PSS must comply with the specifications provided. It is ultimately the operator’s responsibility to make sure that equipment fitted to vehicles is suitable for it’s intended purpose." TfL statement "TfL does not have the power to regulate the equipment market, and we consider that the technical specification and operator’s guidance provide sufficient details for operators to choose compliant equipment. TfL proactively enforces the HGV Safety Permit Scheme through roadside enforcement checks, and should any equipment be found to be non-compliant a PCN may be issued."
