Designing for wellbeing calls on us to seek uncommon partners and span the boundaries that might silo us. For #WellbeingWednesday this week, we look at the intersection of the medical system, art and wellbeing. Pepón Osorio created an art installation that centers the experiences of patients of color. Each patient's story highlights how inequities in the medical system are reflected in patients' access to wellbeing.

With expensive C-suites, bureaucracy and software that add little or negative value while costing a fortune, dinosaur hospitals can’t compete on value per cost. Some surgeons and I have restructured OR processes and scheduling to pull small hospitals out of the red while the CEO and CFOs were clueless as to why things were going better. Everyone working in the ORs knew why because they were part of it and contributed clinical domain knowledge for efficiency, productivity and safety. A large hospital. If run well, should be able to compete on cost and convenience. But, they often can’t differentate good teams from bad teams. I’ve worked with both and seen bad anesthesia groups last for years before being booted out when a good CEO finally arrived. Bad teams can’t easily hide in outpatient centers. Good, agile teams in hospitals can out compete many free-standing centers on capacity utilization with the right people in charge. So… it’s the people that make the difference more than the size of the hospital. Bad leaders and teams can’t hide in small facilities for long. Twice...I’ve seen small hospitals collapse in less than a year when key team leaders have left.
Plenty of high margin treatments (orthopedic, oncology and ENT) were forever lost to the outpatient arena. But also long-term (maybe permanent) drops in "demand" for appendectomies (antibiotics are being successfully used in more cases), GYN oncology surgeries (maybe HPV vaccines are working), breast-related procedures (maybe cancer screening is down, but nobody knows), neurodegenerative (MS) disorders treatment (maybe diagnostic lags, maybe systemic lack of neurologists) and a significant drop in cholecystectomy volumes (nobody knows why). How should #hospitals respond to decreasing demand for sickcare? #healthcarecosts #tripleaim #capitation
Will hospital volumes ever return to what they were?

?? #RecommendedReading: In acknowledgment of #BlackHistoryMonth and #HeartMonth, we explore a recent real-world study co-written by ALPS founding president, David Armstrong, shedding light on racial disparities in PAD diagnosis, treatment, and outcomes. Notably, Black patients had a 50% lower chance of receiving limb-saving interventions. "The bottom line is that people of color – even when insured – were nearly 50% less likely to receive potentially limb-saving vascular intervention. They consequently were more likely to have a stroke, heart attack, or major amputation. We can do better," said our founding president, David Armstrong. #PD #enhancedrehabilitationmatters #motoroutcome #exercise "We found that the subjects who underwent specialized enhanced rehabilitation had a better motor outcome over time than those who were managed by expert neurologists but had participated in community programs for exercise and other allied health interventions. The greatest effects were seen in patients in the early stages of the disease with a high amount of vigorous exercise per week in the last six months. Similar effects were seen for PDQ39, MCSI, the number of falls, and hospitalization. Conclusions: Long-term benefits to motor function and the quality of life in patients with PD and burden reduction in their caregivers can be achieved through a systematic program of specialized enhanced rehabilitation interventions."
