In the NHS they all say: 1. "Of course, we prioritize teaching juniors. They’re the future of surgery, after all." 2. "Don’t worry, you’ll get hands-on experience soon—once things quiet down." 3. "Surgeons should always remain calm, no matter how tough the situation is." In reality: 1. "I’m too busy for teaching today, just shadow for now." 2. "We’ll do training next time… or the time after." 3. Snaps at the team during a stressful moment “Why is no one keeping up? Get it together!” It’s time we move beyond the talk. True #leadership in the #NHS means prioritizing training and #mentoring, even when it’s inconvenient, and managing stress without making it everyone else’s problem. Leadership is about setting the example, not just preaching it. Real change begins with action, not words. Disclaimer: I have worked with some brilliant surgeons, who have mastered a balanced approach towards training and time management, but on the other hand, majority of the NHS teams show a toxic leadership traits.

In my journey as an orthopedic surgeon, I often reflect on the complexities of our healthcare system. For years, I felt a growing frustration as I navigated the limitations of working within a hospital framework. When I was in private practice, I had the autonomy to implement changes that directly benefited my patients. If I noticed something that needed improvement, I could act swiftly. However, once I transitioned to a hospital-owned practice, I quickly realized that this freedom was stripped away. Decisions were delayed, meetings were scheduled just to discuss future meetings, and I found myself in a constant state of waiting—waiting for changes that often never materialized. This stagnation was not just a professional challenge; it was a personal one. I was overwhelmed, facing the pressure to see more patients with fewer resources, all while maintaining the same level of care. It was unacceptable. I felt like I was drowning under the weight of bureaucracy, and it was impacting my ability to serve my patients effectively. At the height of this struggle, I had a small child at home, and the demands of my job, coupled with being on call at all hours, left me feeling utterly drained. One day, while I was grabbing a sandwich at Togo’s, I unexpectedly ran into my nurse, who was on maternity leave. Her absence highlighted how vital our collaborative system was, and without it, everything felt chaotic. In that moment, I realized I couldn’t continue down this path. I needed to reclaim my autonomy and create an environment where I could truly serve my patients. So, in a moment of clarity, I decided enough was enough. I looked at my nurse and said, “Funny story, I quit.” This decision marked a turning point in my career. I was ready to embrace a new chapter—one where I could lead, innovate, and make impactful changes without the constraints that had been holding me back. As healthcare professionals, we must advocate for the changes we want to see. Our patients depend on us to create a system that works—not just for the institution, but for their wellbeing. Private surgical practices are an area ripe for innovation and change. Over the years we’ve had the privilege of supporting some incredible surgeons across different specialties (vascular surgery, orthopaedic, neurology, podiatry, GI, OBGYN and cosmetic surgery) who have operated their practices at Lina for years. While the challenges of surgical private practices may be complex with low reimbursement rates, prior authorizations, all the pitfalls of billing and coding and more. This lack of accessible support can lead to loneliness and isolation, sometimes the best help comes from peer-to-peer connections. Better Health's Peer Support program is so powerful because each coach has walked in the member’s shoes, having experienced the same medical condition that allows for hands-on guidance, support, and encouragement. The result: 86% of our members that use our Peer Support program are able to avoid ER visits which leads to a reduction in overutilization of care and costs.

Enjoyed the AAHKS Annual meeting (as always). My talk at the Business of Arthroplasty last Friday focused on the basics of performing joint replacements in an ASC. Take home points: ?? ASC ownership has gone from a "nice to have" to a "must have" for Orthopedic Surgeons who want to remain competitive and independent in the current environment. ?? Plenty of studies demonstrating the safety of outpatient TJA in an ASC for properly selected patients. I suspect we'll see the indications continue to expand as optimization protocols improve and we collect more outcomes data on higher acuity patients. ?? Understanding the regulatory issues is critical. Safe harbors to the Anti-Kickback Statute allow for TJA in an owned ASC, but there are conditions to be met. CON laws remain a barrier for some. ?? TJA in an ASC is a win-win-win scenario. Surgeons benefit from greater autonomy and ability to capture value creation. Patients get a customized experience, often in a specialized center. The system wins too -- ASC TJA often costs tens of thousands of dollars less. ?? There are many different models (JVs, sole physician-owned, physicians + management company) but having a clear picture of the financials is also important. Payor mix, case mix, volume, equipment needs, staffing, work flows, implant pricing -- they all play a part in ensuring success. ?? While CMS is again scheduled to cut physician professional fees in 2025, ASC facility fees are going up by 2.9% next year.
