Iris nevus: Like iris freckles, iris nevi are dark spots on the colored iris. However, iris nevi are larger than iris freckles, and they grow down into a part of the iris called the stroma. I am surprised that this patient remained undiagnosed up until this age. There is a need to utilise multi-modality cardiovascular imaging in this case. Here are 4 pathologies I spotted from this scan: 1) A uni-ventricular heart: a congenital cardiac condition in which one of the ventricles is underdeveloped or non-functional, leading to a single functional ventricle. This anomalous structure typically results in the mixing of oxygenated and deoxygenated blood, resulting to systemic hypoxemia and cyanosis. 2) Double-inlet left ventricle: both atria connect to the left ventricle, which receives blood from both the systemic and pulmonary circulations. This condition presents challenges to effectively managing blood flow and can lead to heart failure and other complications. 3) Aneurysmal atrial septum: typically indicates a weakened area in the wall that separates the left and right atria, potentially causing turbulent blood flow and increasing the risk of thromboembolic events. This anomaly can also affect atrial function and add to the overall complexity of managing the condition.
36 year old woman presented in the E.R. with dyspnea-orthopnea and inability to walk. The echocardiography was… unbelievable. ( The second day after the admission, in better shape , the patient revealed that it was diagnosed in childbirth and had same medical treatment, although when she came in the ER, we didn’t expect that).
