In Fiscal Year 24, a nurse-led Ambulatory Practices Falls Subcommittee was established to help educate ambulatory caregivers on best practices to reduce patient falls in ambulatory settings. Their work included sharing weekly Huddle Tips on fall prevention, disseminating a Falls Best Practice tip sheet to all outpatient practices, and creating a virtual best practice training reference guide. Thanks to their expertise and guidance, the nurse-driven effort resulted in a 25% decrease in falls. #NursingExcellence #ExceptionalOutcomes #QualityAndSafety

Patient identification errors can lead to wrong treatments, missed procedures, and compromised care. Here’s how we can prevent them: 1. Using multiple identifiers like UHID number and birthdate. 2. Implementing standardized protocols for confirming patient details. 3. Enabling electronic alerts to flag similar names. 4. Providing ongoing staff training on identification best practices. 5. Involving patients in actively confirming their details.
