Combating Note Bloat in Medical Documentation ? Did you know that the average length of medical notes has increased by 8% in the last three years and nearly doubled over the past two decades? While this might not be surprising, the persistence of 'note bloat'—lengthy notes crammed with unnecessary details—continues to complicate medical documentation, burdening clinicians and undermining efficient clinical care. The root of this issue can be traced back to outdated documentation standards that required extensive recording of patient symptoms, regardless of their relevance to the current visit. Despite recent changes in billing regulations designed to streamline documentation, the problem of note bloat not only persists—it's intensifying. At River Records, we are dedicated to eliminating excessive documentation. We are developing a system that not only simplifies note creation but also organizes information to more accurately reflect the clinician's perspective on patient care. ? Curious to see how we're tackling this challenge? One of my favorite quotes from a physician I worked with was, “No one asked me if I wanted a third party in the exam room with my patient.” She was referring to the electronic medical record (EMR) system and the various challenges and disruptions it brought to her patient care and work. In a previous role, we documented physicians having to login to as many as 34 different accounts at various points in their workflow. If your patient care, workflow, or revenue cycle solution requires yet another account login, consider the impact it will create for your physicians and those they care for.
